Cost analysis of leak after sleeve gastrectomy

Abstract: Leaks after LSG are an expensive complication. It is therefore important to take all necessary measures to reduce their incidence. Our data should be considered when analyzing the cost effectiveness of staple line reinforcement usage.

“…Indirect costs, including duration and cost of sick leave and outpatient costs related to home care, were not evaluated, as they are likely to be attributed to the presence of gastric leak rather than the type of stent used. In another recent cost‐effectiveness analysis of gastric leak after sleeve gastrectomy, Nedelcu and colleagues did incorporate indirect costs, which may explain the mean higher costs reported (€75 682 versus €22 470 in the present series). Another explanation for the discrepancy could be the number of patients included, as patient management and therefore the inherent costs are influenced by the experience of the endoscopy team.…”

“…This study was not designed to evaluate costs in relation to leak or bleeding complication. It is known that leaks are extremely costly, and for example, can result in prolonged hospitalization within an intensive care unit as well as additional outpatient costs [172]. Since bleeding complications can be associated with leaks, data regarding bleeding would have been an asset.…”

“…This is associated with increased economic burden of the operation. 32 In this study, the median time until healing of leak was 43 days and the median duration of hospital stay was 50 days, and finally 2 patients died with a mortality rate 10%. Comparable to these outcomes, Rebibo 18 et al have reported 1.2% mortality rate and the median time to healing gastric leak was 84 days.…”
