A Systematic Review of Model-Based Economic Evaluations of Treatments for Venous Leg Ulcers

Objective The aim of this review was to identify, and assess the quality of, published model-based economic evaluations relating to treatments for patients with venous leg ulcers to help inform future decision-analytic models in this clinical area. Methods A systematic literature search was performed on six electronic databases, from database inception until 21 May 2018. Search results were screened against predefined criteria by two independent reviewers. Data was then extracted from the included studies using a standardised form, whilst the decision-analytic model-specific Philips Checklist was used to assess quality and to inform model critique. Results A total of 23 models were identified, 12 studies used a Markov modelling approach, five used decision trees and six studies did not detail the model type. Studies were predominantly from the National Health Service (NHS)/payer perspective, with only two taking a societal perspective. Interventions were wide ranging, but dressing technologies (11/23) were most common. The intervention studied was found to be dominant in 22/23 studies. The reporting quality of papers was mostly low, with evidence behind model structures, time horizons and data selection consistently underreported across the included papers. Conclusions This review has identified a sizeable literature of model-based economic evaluations, evaluating treatments for venous leg ulcers. However, the methods used to conduct such studies were generally poorly reported. In particular, the reporting of evidence surrounding the model structure, justification of the time horizon used and the rationale for selecting data inputs should be focused on in any future models developed.

Estimating the economic value of emerging technologies in clinical medicine presents a number of problems. New technologies may have a limited clinical history, few supportive peer‐reviewed publications, and only anecdotal evidence as they enter the market and seek clinician approval and reimbursement from payers. Although clinical efficacy/effectiveness research is a minimal starting point for making the case for adoption of a product, establishing a competitive cost‐effectiveness position against other products and establishing the case for economic value must be made as well when presenting to health plans or other payers. Economic valuation methods have been well developed in the business community. Reviewed here are the components of a well‐crafted case for the economic value of a product in general and in the wound industry specifically, in a multidomain approach to demonstrate values using demographic, clinical, financial, operational, and intangible assessments.

RESUMO: Objetivo: investigar o sentimento de inclusão social de pessoas com úlcera venosa. Método: pesquisa qualitativa realizada com oito pessoas com úlceras venosa, atendidas em um ambulatório de curativos, em João Pessoa-Paraíba, Brasil. A coleta de dados realizada em fevereiro de 2012, ocorreu por meio de entrevista estruturada, com exploração dos discursos pela técnica de análise de conteúdo temática. Resultados: as pessoas com úlcera venosa percebem a lesão como um sofrimento, preferem atividades de lazer do tipo exclusivas como assistir à televisão, algumas se sentem incluídas na sociedade e outras sofrem com o estigma de ter a lesão, afastadas do convívio de familiares. Conclusão: faz-se necessário repensar práticas assistenciais inclusivas e receptivas que transcendam a técnica de curativo e compreendam o ser humano em sua complexidade.
