Cost of the Cervical Cancer Screening Program at the Mexican Social Security Institute

Abstract: Objective. To estimate the annual cost of the National Cervical Cancer Screening Program (CCSP) of the Mexican Institute of Social Security (IMSS). Materials and methods. This cost analysis examined regional coverage rates reported by IMSS. We estimated the number of cytology, colposcopy, biopsy and pathology evaluations, as well as the diagnostic test and treatment costs for cervical intraepithelial neoplasia grade II and III (CIN 2/3) and cervical cancer. Diagnostic test costs were estimated using a micro-co… Show more

“…After colposcopy study, 51% of samples were reclassified as LSIL. Granados-García et al 17 found false negative PAPs in 42.6% of samples, while we found 26.66% of false negatives (Table 2). Altogether, these results suggest that colposcopy is a better assay than cytology to predict HPV infections causing lesions in the histopathology assay.…”

“…With this background, in the context of countries such as Mexico, the operative problems of the population's utilization of the Pap test are magnified including non-optimal practices for cytology specimen collection, deficient fixation and staining and, most particularly, lacking competencies in cytology diagnosis as evidenced by this study (38). Therefore, it is necessary to change the paradigm of using the Pap test, favoring its high specificity, and to find the evidence of its greater efficiency when used as a triage strategy in HPV-positive women.…”

“…[ 2 3 ] The use of these different screening methods depends on the organization of the local health system and the resources available. [ 2 3 ] Because the material resources to identify cervical preinvasive lesions can consume large amounts of the institutional budget,[ 4 5 ] it is essential to make screening more effective and efficient.…”
