Cost-utility Analysis of Trabecular Micro-bypass Stents (TBS) in Patients With Mild-to-moderate Open-angle Glaucoma in Italy

Abstract: BackgroundGlaucoma is a disease characterized by progressive damage of the optic nerve. Several therapeutic options are available to lower intraocular pressure (IOP). In primary open-angle glaucoma (POAG) patients with inadequate IOP control (or controlled with multiple medical therapies or for whom medical therapy is contraindicated), the implantation of micro-invasive glaucoma surgery devices (MIGS) and concomitant cataract surgery has proved to be more effective in reducing intraocular pressure (IOP), as co… Show more

“…Results of this economic evaluation suggest that use of TBD stents compared with usual care in patients with primary OAG is cost effective, with an ICER of A$2430 in the cataract surgery setting and an incremental cost per QALY gained of A$14,644 in the standalone setting. Similar or more favourable findings were identified in recently published literature in European and Canadian health care settings for mild to moderate OAG in both the cataract surgery [33][34][35] and standalone populations [36].…”

“…Along with dramatic increases in MIGS volumes, the associated economic impact to the larger healthcare system must be considered. Using efficacy data obtained from trabecular microbypass (iStent) pivotal trial, Fea and colleagues performed a cost-utility analysis to assess cost-effectiveness of combined iStent and cataract surgery versus cataract surgery alone from the perspective of the Italian National Healthcare Service [12 ▪▪ ]. The investigators employed a Markov model that estimated clinical benefits (measured in quality-adjusted life years, QALY) and costs associated with each treatment arm.…”
