Costs of Testing for Ocular Chlamydia trachomatis Infection Compared to Mass Drug Administration for Trachoma in The Gambia: Application of Results from the PRET Study

Abstract: BackgroundMass drug administration (MDA) treatment of active trachoma with antibiotic is recommended to be initiated in any district where the prevalence of trachoma inflammation, follicular (TF) is ≥10% in children aged 1–9 years, and then to continue for at least three annual rounds before resurvey. In The Gambia the PRET study found that discontinuing MDA based on testing a sample of children for ocular Chlamydia trachomatis(Ct) infection after one MDA round had similar effects to continuing MDA for three r… Show more

“…Numerous nucleic acid-amplification tests (NAATs) have been developed, including the adapted use of commercial kits originally designed for diagnosing genitourinary Ct infections [1216]. NAATs have been shown to be cost-effective in some settings [17] but concerns have been raised that the per-sample cost of NAATs can be too much for national eye health programmes in countries where trachoma remains a problem [18]. The cost of specialist devices and platforms for deploying NAATs can also be prohibitive.…”

“…[11][25] More evidence is needed on the cost of post-mass treatment surveillance, including Transmission Assessment Surveys, and certification of eradication or elimination. Indeed, we are not aware of any studies of the cost of eradication or elimination of diseases (including guinea-worm disease or poliomyelitis) that explicitly included the cost of the certification process.…”

“…The Gambia is divided into geographically-defined census Enumeration Areas (EAs) of approximately 600-800 people each. Sampling by EA is equivalent to sampling settlements with probability proportional to their size 53 . Twenty EAs in each of URR and LRR were randomly selected for participation.…”
