COVID-19 and hospital management costs: the Italian experience

Abstract: Background This article investigates the hospital costs related to the management of COVID-19 positive patients, requiring a hospitalization (from the positivity confirmation to discharge, including rehabilitation activities). Methods A time-driven activity-based costing analysis, grounding on administrative and accounting flows provided by the management control, was implemented to define costs related to the hospital management of COVID-19 positi… Show more

“…Carrera-Hueso et al (2021) estimated total hospitalization costs from a hospital perspective in Spain, respectively per non-ICU patient and per ICU patient at € 60,997 and € 341,845 (adjusted to German 2021 euros, using harmonised indices of consumer prices and purchasing power parities from Eurostat [ 22 , 49 ], rounded to whole euros) [ 17 ]. Moreover, in a study including six public health hospitals in Italy total per patient hospitalization costs from a hospital perspective were € 6,668, € 9,188 and € 18, 275 (inflated to German 2021 euros [ 22 , 49 ], rounded to whole euros), for respectively low-complexity care, medium-complexity care and high complexity care of COVID-19 patients [ 50 ]. However, considering the different health-care systems between different countries we consider these cost to be incomparable.…”

“…Only one other study looked at daily hospitalization costs in western Europe. Foglia et al (2022) estimated daily hospitalization costs from a hospital perspective at € 546, € 804 and € 1610 (inflated to German 2021 euros [ 22 , 49 ], rounded to whole euros), for respectively low-complexity care, medium-complexity care and high complexity care using data from six public health hospitals in Italy [ 50 ]. Other researchers estimated the costs of a day in the ICU in Germany for non-COVID-19 patients between €744 and €1,462 (inflated to 2021 [ 22 ], rounded to whole euros) [ 23 – 28 ].…”

“…Our cost estimates associated with COVID-19 hospitalisation are similar to those used by the National Institute for Health and Care Excellence Technology Appraisal assessing therapeutics for people with COVID-19, although the report restricted to severe COVID-19 patients and used non-comparable methodology 42. Data from other countries, for instance, a retrospective study of hospitalised COVID-19 patients in the USA from 1 April to 31 December 2020 using claims data, estimated the average cost per day for overall admissions and ICU admissions was $1772 and $2902, respectively,43 with evidence from Italy reporting the hospitalisation cost per day varying based on the complexity of care (low complexity=€476; medium complexity=€700; high complexity=€1402) 44. However, the generalisability of these estimates to the UK population is limited given differences in populations, data coverage period, COVID-19 management strategies and healthcare systems.…”

Notably, due to variability within the relatively smaller immunocompromised and hospitalised, it is likely that our findings related to costs in this group differ from existing studies.…”

“…Knowledge of treatment cost for COVID-19 inpatients in the real word is greatly needed to better improve the COVID-19 treatment guidelines and to inform scarce financial health resource planning, in particular for health insurance fund arrangements and public finance budget decisions ( 5 , 6 ). Yet, few published studies have focused on examining the COVID-19 treatment cost by using data derived from actual COVID-19 patients, and most of those are from the United States of America (USA) ( 5 , 7 9 ).…”
