COVID‐19 associated Kikuchi‐Fujimoto disease

Abstract: Summary We discuss the case of a 17‐year‐old male who presented with painful lymphadenopathy, fevers, weight loss, anorexia and a history of preceding anosmia. Histological features on lymph node biopsy were in‐keeping with Kikuchi‐Fujimoto disease (KFD). We later confirmed SARS‐CoV‐2 (COVID‐19) infection on antibody testing. This case describes an atypical presentation of COVID‐19 in a young patient and highlights an association between the virus and this rare disease.

“…In addition, KFD can occur post or concurrent with COVID-19 infection [15,16]. While the ethnicity of the patients was not specifically mentioned in the available case reports, the patients were young (less than 20 years old), presented with prolonged fever and other systemic symptoms and one case mentioned rapid resolution of the symptoms within 7 days of commencing supportive treatment [15,16]. The clinical course of these cases was similar to that of KFD in general, as well as the cases of KFD post COVID-19 vaccination.…”

“…The etiopathogenesis of KFD remains unclear; an autoimmune response and diverse infectious agents are the most common hypotheses [ 1 , 2 ]. More recently in the coronavirus disease 2019 (COVID-19) pandemic, few cases of KFD after infection have been reported [ 3 , 4 ]. However, to our knowledge, only four cases of KFD following vaccination against COVID-19 have been described to date [ 5 - 7 ].…”

“…9 Reoccurrence has been recorded after a period of 2-10 years of initial presentation. 10 Various atypical presentation or association of KFD has been reported such as high levels of circulating Epstein-Barr viral DNA 11 , lymphocytic meningitis, 12 autoimmune hemolytic anemia, 13 Corona virus disease-19, 14 relapsing polychondritis, 15 acute appendicitis, 16 periportal and retroperitoneal lymphadenopathy, 17 fever of unknown origin, 18 and acute renal failure. 19 The consequence of various nonspecific clinical presentations of KFD is a broader differential diagnosis.…”
