Objective: To determine the pattern of COVID-19 on chest radiograph in patients presenting to Lady Reading Hospital, Peshawar, Pakistan. Methods: This prospective observational study was conducted on 178 consecutive swab positive COVID-19 patients presenting to Lady Reading Hospital, Peshawar, Pakistan from 15th March to 15th June 2020. Patients of all ages and both genders were included. Chest X-rays performed by portable radiography unit were viewed for different patterns by two consultant radiologists independently and results were analyzed using IBM SPSS 20. Results: Out of 178 patients 134 were male. Mean age was 55.67 years. Radiographic patterns observed were ground glass haze without or with reticulation and/or consolidation (45.5 % and 33.2% respectively) and predominant consolidation either alone or in combination with ground glass haze or other findings (27.1% collectively). Peripheral distribution pattern was seen in 69.1% of patients with bilateral findings in 84.3%. Further categorization was based on pulmonary zonal demarcation with changes most commonly involving four zones (33.1%) i.e., the lower and mid zones bilaterally. Conclusion: Portable chest radiography is an essential supporting tool for assessing different patterns in COVID-19 infection. Objectives: The Dutch Radiological Society developed CO-RADS classification, a system for the classification of CT scan chest findings among suspected COVID-19 patients. However due to some important issues it was modified by authors and then applied on our study population. The objective was to study the spectrum of lungs involvement as concluded by HRCT scan chest finding and classifying it using the “Modified CO-RADS classification” Methods: This cross-sectional study was conducted jointly by the departments of Medicine and Radiology, JPMC from January 16, 2021 to April 30, 2021. This study includes suspected cases of COVID-19 patients aged between 18-80 years who came for HRCT chest. Their data variables were recorded. HRCT findings were classified using “Modified CO-RADS classification”. Patients’ results of real time PCR for COVID-19 were also followed. Results: A total of 78 patients presented to the study department during this study period. Of them 85.8% were male (n=67) and 14.2% were female (n=11). Out of the 78 patients, 58 were tested positive for COVID-19 on first RT-PCR on follow up. Among positive two patients (3.4%) had CO-RADS-1, 4 patients (7%) had CO-RADS-2, 19 patients (32.75%) had CO-RADS-3, 21 patients (36.2%) had CO-RADS-4 while 12 patients (20.7%) had CO-RADS-5 category. (CO-RAD-6 category was omitted). Of the patient who had negative results on RT-PCR, five patients had CO-RADS-4 while three patients had CO-RADS-5. On repeat RT-PCR all (8/8) patients of category IV and V proved Covid-19 positive. Conclusion: HRCT scan chest can be used for quicker diagnosis of COVID-19 patients in patients with respiratory complaints in whom prompt diagnosis is required and when RT-PCR investigation process would be taking prolonged time due to over burden during pandemic situation. “CO-RADS classification after modification” proved more effective communicative tool to label and understand the severity of lung involvement in Covid-19 disease. doi: https://doi.org/10.12669/pjms.38.4.4687 How to cite this:Tabassum S, Haider S, Shaukat S. Objective: To determine the accuracy of high-resolution CT (HRCT) in early diagnosis of COVID-19 in comparison to Reverse-Transcription Polymerase Chain Reaction (RT-PCR). Study Design and Setting: This was a retrospective study, conducted in Department of Radiology, PNS Shifa Hospital, Karachi from April 2020 to June 2020. Methodology: A total of 115 patients were included in the study who presented with symptoms. Patient’s data such as demographic details, clinical presentations, RT-PCR reports and HRCT reports were collected from the record of the hospital. Data was entered and analyzed using SPSS-25. Results: Out of 115 study participants, 98(85.2%) were male and 17(14.8%) were female with the average age of 50.53 (±18.64) years. Fever along with cough was the most common clinical presentation, presented by 91(79.1%) subjects. In comparison to RT-PCR, sensitivity of HRCT was 96.2%, its specificity standing at 60% with the overall accuracy of 93.19%. Moreover, mortality Rate of these symptomatic patients was 11.30% (13 out of 115). The average age of the expired patients was 67.07 (±11.45) years and the age of discharged patients was 48.31 (±17.92) years. The difference in age was statistically significant (p-value <0.001). Conclusion: Highly specific and sensitive diagnostic tests for COVID-19 have not yet been developed. High resolution CT chest has high sensitivity for early diagnosis of COVID-19, hence HRCT should be incorporated as a primary tool in assessment criteria of hospitalized patients for early diagnosis of COVID-19.
