COVID-19 Outbreak and Surgical Practice

Abstract: Little is known about surgical practice in the initial phase of coronavirus disease 2019 (COVID-19) global crisis. This is a retrospective case series of 4 surgical patients (cholecystectomy, hernia repair, gastric bypass, and hysterectomy) who developed perioperative complications in the first few weeks of COVID-19 outbreak in Tehran, Iran in the month of February 2020. COVID-19 can complicate the perioperative course with diagnostic challenge and a high potential fatality rate. In locations with widespread i… Show more

“…that compared with other complications, post-operative COVID-19 led to longer hospital stay and more frequent need for mechanical ventilation in the post-operative period. These results agree with previously published studies [2][3][4]. The one-month mortality did not differ substantially between COVID-19 and non-COVID-19 patients, but it is possible that the younger age of the first group in our cohort may have been a protective factor.…”

“…In the post-operative period, COVID-19 may also be associated with a poorer prognosis [2][3][4] because of factors such as general anesthesia and mechanical ventilation, blood loss/transfusion, and systemic inflammatory response syndrome associated with aggressive surgical management [5]. The diagnosis can also be challenging because symptoms of COVID-19 may be attributed to other common postoperative conditions.…”

“…También desde este mismo día en el Servicio de Cirugía General del Hospital Universitario La Paz se suspendieron todas las reuniones presenciales y se realizaron los comités multidisciplinares de forma telemática. Así mismo se suspendieron las intervenciones quirúrgicas programadas por patología benigna (10) , se cancelaron las consultas no oncológicas del servicio y se extremaron las medidas de precaución entre el personal sanitario y administrativo del hospital.…”

“…Where there is wide community spread of COVID-19, deferring elective surgery is highly recommended. The Iranian series by Aminian et al [1] speaks about COVID-19 complicating the perioperative course with diagnostic challenges and a high potential post-operative fatality. The other approach would be to screen everyone planned for elective surgery; however, this is a very resource and labor intense process and even then, may not be fool-proof as there are varying reports on the sensitivity, specificity, positive predictive value and negative predictive values of the available COVID-19 tests.…”
