South Africa has recorded the highest COVID-19 morbidity and mortality compared to other African regions. Several authors have linked the least amount of death in African countries with under-reporting due to poor health systems and patients’ health-seeking behaviors, making the use of clinical audits more relevant for establishing the root causes of health problems, and improving quality patient care outcomes. Clinical audits, such as mortality audits, have a significant role in improving quality health care services, but very little is documented about the outcomes of the audits. Therefore, the study sought to determine the health care system and patient-related factors associated with COVID-19 mortality by reviewing the COVID-19 inpatient mortality audit narration reports. This was a retrospective qualitative research approach of all hospitalized COVID-19 patients, resulting in death between the first and second COVID-19 pandemic waves. Thematic analysis employed inductive coding to identify themes from mortality audits from all 41 public hospitals in Limpopo Province, South Africa. Four themes with seventeen sub-themes emerged: sub-standard emergency medical care provided, referral system inefficiencies contributed to delays in access to health care services, the advanced age of patients with known and unknown comorbidities, and poor management of medical supplies and equipment, as a health system and patient-related factors that contributed to the high mortality of COVID-19 patients. There is a need to routinely conduct clinical audits to identify clinical challenges and make recommendations for health promotion, risk communication, and community engagement. We recommend reviewing and expanding the scope of practice for health-care providers during epidemics and pandemics that include aspects such as task-shifting.

Over the recent time period, pediatric allergy clinics across the world have markedly changed their practice because of the COVID-19 pandemic. Nowadays, clinics are not inclined to accept a patient demanding a new procedure / therapeutic modality during pandemic. All allergic diseases require continuous management and treatment, and their socioeconomic burden has been increasing worldwide. In this chapter, the aim is to focus on allergic diseases management during pandemic. During this time, patient follow-up, patient management, and diagnostic tests are real challenges. Limited face-to-face consultations and as much as use of telemedicine are currently seen as the major issues in the allergy practice. Face-to-face examination and treatment should be preferred only in vital situations. During COVID-19 pandemic, patient education, which is the most important step in the treatment of allergic diseases, has started to be done online. The prevailing opinion in the allergy community is that the treatment should not be interrupted, or dose reduction should not be made. According to the guidelines, it is appropriately recommended to carefully calculate the profit and loss of the treatment on a case-by-case basis.

COVID-19 in children with bronchial asthma: aspects of comorbidity

Relevance: to date, there is no single view on how COVID-19 proceeds in children with bronchial asthma. At the same time, bronchial asthma is observed in 14% of children in the population and their number continues to increase. COVID-19, despite the end of the pandemic, also remains one of the most common infections, and therefore the features of its course in comorbid children are of interest. Objective: to determine the features of the clinical course, laboratory and instrumental examination data, and to analyze the effectiveness of COVID-19 treatment in children with bronchial asthma. Materials and methods: 25 children with COVID-19 who suffered from bronchial asthma (observation group) and 25 children with this infection without asthma (comparison group) were monitored. All patients were examined in accordance with the current regulatory documents on COVID-19, followed by statistical processing of the results. Results: cough and shortness of breath were observed more often and longer in comorbid patients. Viral lung damage with the development of mild respiratory failure was more common in children with bronchial asthma. In this group, there was a more pronounced tendency to hypercoagulation and systemic inflammatory response relative to patients in the comparison group. At the same time, against the background of adequate treatment, which turned out to be significantly longer in all types of therapy, with the exception of anticoagulant, coagulogram parameters and protein levels of the acute phase of inflammation in the dynamics normalized in both observed groups. As a result, patients with COVID-19 and bronchial asthma spent significantly longer in hospital than children who did not have a anamnesis of bronchial asthma. Conclusion: COVID-19 in children with bronchial asthma was somewhat more severe compared to those who did not suffer from bronchial asthma. Cough and shortness of breath were clinically prevalent, symptoms characteristic of both diseases, but there were no typical exacerbations of asthma in this group.
