Creating and Validating an Algorithm to Measure AIDS Mortality in the Adult Population using Verbal Autopsy

Abstract: BackgroundVital registration and cause of death reporting is incomplete in the countries in which the HIV epidemic is most severe. A reliable tool that is independent of HIV status is needed for measuring the frequency of AIDS deaths and ultimately the impact of antiretroviral therapy on mortality. Methods and FindingsA verbal autopsy questionnaire was administered to caregivers of 381 adults of known HIV status who died between 1998 and 2003 in Manicaland, eastern Zimbabwe. Individuals who were HIV positive an… Show more

“…[11] In SA, death certificates rarely define AIDS mortality and retrospective clinical data are limited. [12] Cause of parental death was therefore determined using the verbal autopsy method, [13] validated in SA with sensitivity of 89% and specificity of 93%. [14] Determination of AIDS-related parental death required a conservative threshold of ≄3 AIDS-defining illnesses; e.g.…”

“…Study staff also conducted a verbal autopsy for each patient. Verbal autopsies are a method for assessing causes of deaths, including those related to TB and HIV, in resource-limited countries (27)(28)(29)(30)(31)(32). They involve interviewing a family member or friend who was closely associated with the patient during the period preceding death.…”

“…The original study had a sensitivity of 66% and specificity of 76% of predicting death due to AIDS; sensitivity and specificity did not vary significantly according to gender, time of death, and whether the respondent was a primary caregiver, family member, or other relation to the deceased [40,41]. Verbal autopsy validation in Ghana also compared assigned causes of death from verbal autopsy to medically certified causes of death [40] and was found to correctly identify 78% of deaths. Verbal autopsy has since been used in several studies with high predictive powers [42][43][44].…”
