Mortality patterns in Southern Adriatic islands of Croatia: a registry-based study

AimTo investigate the mortality patterns on the Southern Adriatic islands of Croatia and compare them with those in two, mainly coastal, mainland counties.MethodsIn this registry-based study we used the official mortality register data to analyze the mortality patterns on seven Croatian islands (Brač, Hvar, Korčula, Lastovo, Mljet, Šolta, and Vis) and Pelješac peninsula in the 1998-2013 period and calculated the average lifespan, life expectancy, and standardized mortality ratios (SMR). We compared the leading causes of death with those in the mainland population of two southernmost Croatian counties.ResultsThe average lifespan of the island population was 3-10 years longer for men and 2-7 years longer for women than that on the mainland. All-cause SMRs were significantly lower for both men and women on Korčula, Brač, Mljet, and Pelješac but significantly higher for women on Šolta (1.22; 95% confidence intervals 1.07-1.38). The leading causes of death on the islands were cardiovascular diseases, with higher percentages in men and lower in women in comparison with those on the mainland. There were no substantial differences in the life expectancy at birth.ConclusionsDespite longer lifespan, lack of differences in life expectancy at birth suggests that the recent generations of islanders no longer show beneficial mortality patterns, possibly due to diminishing adherence to the Mediterranean diet and lifestyle. Restoring the traditional lifestyles is a public health priority, with the ultimate aim of reducing inequalities and improving the health of island inhabitants.

Transplantacija organa-etika, zakonodavstvo; Pretpostavljeni pristanak-etika; Informirani pristanaketika; Dobavljanje tkiva i organa-etika, zakonodavstvo; Darovatelji organa; Moždana smrt; Odabir pacijenata-etika; Usmjereno darovanje organa; Hrvatska Sažetak. Transplantacijska etika (TE) određena je brojnim povijesnim, fi lozofskim, znanstvenim, medicinskim, pravnim, sociološkim i ekonomskim čimbenicima. Nacionalne posebnosti Republike Hrvatske (RH) u ovom se području temelje na zavidnom uspjehu u transplantaciji (TX) solidnih organa i uređenoj relevantnoj legislativi koja pogoduje ovom uspjehu. Tako problemi nedostatka organa za TX, doniranja, dodjele i pravedne raspodjele nisu važne teme TEa u RH, ali globalno ostaju vodeća etička područja u TX-u. U RH je na snazi uzorit Zakon o presumptivnom pristanku na doniranje organa nakon smrti (tzv. zakon opt-out, jer se izjašnjavanje traži za "biti izvan-out" mogućih darovatelja). Nepoznavanje zakona opt-out o doniranju organa nakon moždane smrti, vrijedećega u RH, neusklađenost s običajima i odredbom Kodeksa medicinske etike i deontologije o traženju suglasnosti obitelji za doniranje organa nakon moždane smrti te nedovoljno regrutiranje kandidata za TX u nas se nameću kao sporne etičke teme. Budućnost predviđa mogućnost TX-a organom nehumanog podrijetla kao što je onaj uzgojen u svinjskom organizmu, što otvara nova etička područja i za RH i za ostatak svijeta.

Abstract In 2013–2015, a consortium of European scientists – NEWDANUBE – was established to prepare a birth cohort in the Danube region, including most of the countries with the highest air pollution in Europe, the area being one-fifth of the European Union’s (EU’s) territory, including 14 countries (nine EU member states), over 100 million inhabitants, with numerous challenges: big socioeconomic disparities, and a region-specific environmental pollution. The consortium reflects the EU Strategy for the Danube Region Strategy (2010), which identified 11 thematic Priority Areas – one of which is the environmental risks. Birth cohorts have been established in all other areas of Europe and collaborative efforts in promoting maternal and fetal health by minimizing the environmental exposures have been initiated with national, European, and international financial support. A birth cohort in the Danube area could apply the established methodologies for prenatal exposure and birth outcome measurements and establish a platform for targeted health promotion in couples planning pregnancies. The consortium included a strong socioeconomic part focusing on the participant’s active registration of exposures to environmental toxicants and health indicators of disease and wellbeing, combined with investigation of their risk-reducing behavior and interventions to change their lifestyle to avoid the adverse health risks. Willingness to pay for reducing the health risks in children is also proposed to be estimated. Further collaboration and networking is encouraged as the Danube region has several decades of experience and expertise in biomonitoring adult populations exposed environmentally or occupationally. Additionally, some countries in the Danube region launched small-scale birth cohorts encouraged by participation in several ongoing research projects.
