Crohn’s disease complicated by relapsed extranodal Hodgkin’s lymphoma: prolonged complete remission after unmanipulated PBPC autotransplant

Abstract: Summary:Crohn's disease and ulcerative colitis are inflammatory bowel diseases (IBD), which are thought to result from an inappropriate immunologic (autoimmune) response to luminal antibodies. Allogeneic stem cell transplantation (SCT) performed for coincidental diseases is able to cure both leukaemia and Crohn's disease. Autologous SCT is currently performed worldwide for severe autoimmune diseases (SADs) because of its reduced transplant-related mortality (TRM). We report the case of a 30-year-old male patie… Show more

“…8 This is supported by promising reports on single BD patients treated by stem cell transplantation -allogeneic or autologous -due to leukemia or malignant lymphomas. [9][10][11][12] Recently, two patients with CD have been described in whom, in a period of high clinical activity, (CDAI 308 and 267, respectively) aPBSCT+antithymocyte globulin (ATG) resulted in an impressive response lasting more than 1 year. 13 However, there remained a mild to moderate endoscopical and/or histological inflammation.…”

“…To date, 26 IBD cases consisting of 21 CD and five UC patients who underwent HSCT (eight autologous and 18 allogeneic) for cancer have been reported [8,[10][11][12][13][14][15][16][17][18]. Overall, 24 of 27 (88.9 %) patients have achieved clinical remission in the follow-up periods of 0.5 to 8 years.…”

“…42 Four patients suffering from active Crohn's disease 8.5 years (median, range: 4-10) entered unsupported remission states of Crohn's disease for 1.5 years (median, range: 0.8 to 10 years) after transplant. [44][45][46][47] Two of them died from malignancy, while the Crohn's disease remained in remission for 10 and 25 months. 45,46 Four patients suffering from psoriasis for 13, 15 and 20 years were given autologous transplants for comorbid hematologic malignancies.…”

“…34,42,45,46 Five patients enjoy unmaintained remission of their AID for a median duration of 36 months (range: 19 to 120). 41,44,47,49 A patient with large cell lymphoma and hemophilia A with a natural inhibitor of factor VIII experienced a reduction of factor VIII inhibitor from 752 to 1 BU/ml. 39 Two of four patients with SLE relapsed after 12 and 36 months post transplant.…”
