Crospovidone and Microcrystalline Cellulose

Abstract: Crospovidone and microcrystalline cellulose (MCC) are pharmaceutical fillers well known in the pulmonary pathology literature. Fillers are inactive substances incorporated into medications to facilitate drug delivery. By examining 545 consecutive gastrointestinal surgical specimens from 302 patients between September 11, 2015 and October 23, 2015, we identified the fillers in 29 specimens from 26 patients. The control group consisted of an equal number of consecutive site-matched specimens collected during thi… Show more

“…Fillers are widely used in many over-the counter and prescription medications, as well as vitamins and processed food products. The most frequently used are crospovidone and microcrystalline cellulose (MCC), and these have been identified in 0.3-9.0% of gastrointestinal surgical specimens [56,57].…”

“…Medication fillers such as crospovidone have a coral shaped two-toned purple appearance in the gastrointestinal tract, and is not birefringent (Fig. 8 ) [ 43 ]. In contract, microcrystalline cellulose is rod-like or has a flake appearance, is clear, birefringent, and positive with GMS stain (Fig.…”

“…2 Crospovidone has also been detected in the lungs of patients who have aspirated, 3 and more recently as an incidental finding in gastrointestinal specimens, where its extraluminal presence may be used as an indicator of bowel perforation. 4 In this report, we present two clinically annotated cases of histopathologically confirmed cutaneous crospovidone deposition occurring in patients with known histories of injection drug abuse. The suspected etiology in both cases is subcutaneous or intradermal injection of crushed pharmaceutical tablet mixtures, commonly known as "skin popping.…”
