Objective: To investigate the role of severe physical violence during pregnancy (SPVP) between intimate partners in early cessation of exclusive breast-feeding (EBF). Design: A health services survey. The revised Conflict Tactics Scale was used to characterize SPVP; premature breast-feeding cessation was identified using a current status data approach, which was based on the information reported from food recall during the preceding 7 d. The cumulative hazard function was estimated by complementary log-log transformation models, which allowed the ensuing estimation of early breast-feeding cessation rates in different age groups and the ratio of rates of weaning between women exposed and not exposed to violence. Setting: Five large public primary health-care facilities of Rio de Janeiro, Brazil. Subjects: The sample comprised 811 randomly selected mothers of children under 5 months of age who were waiting to be consulted. Results: SPVP is an independent risk factor of cessation of EBF since, after controlling for socio-economic, demographic, reproductive and lifestyle variables, women exposed to violence presented an incidence density that was 31 % higher than those who were not exposed (hazard ratio 5 1?30, 95 % CI 1?01, 1?69). Conclusions: The findings corroborate the hypothesis that SPVP is an important risk factor for EBF. This indicates the need for incentives to adequately train healthcare personnel in dealing with lactating women in order to gain a broader view of breast-feeding beyond the biological aspects of lactation, including the maternal psychological dimension.

OBJECTIVE:To evaluate physical intimate partner violence during gestation as an independent risk factor for low quality of prenatal care. METHODS:A cross-sectional study was carried out at three public maternity wards of the municipality of Rio de Janeiro (Southeastern Brazil). The 528 puerperal women included in the study were selected by simple random sampling from all babies born at term in 2000. Prenatal care information was collected through the pregnant woman's card and face-to-face interviews. The Kotelchuck index was employed to assess the quality of prenatal care. In order to identify violence situations, the Brazilian version of the instrument Revised Confl ict Tactics Scales was used. Non-conditional logistic regression was used to assess the effect of exposure, after controlling for confounding variables. RESULTS:Even after adjustment for socioeconomic, demographic, reproductive, and couple's lifestyle variables, physical intimate partner violence during gestation remained associated with low quality of prenatal care. Women exposed to physical violence during gestation had 2.2 times more chance of presenting inadequate prenatal care compared to those without history of physical violence. CONCLUSIONS:These fi ndings point to the need of identifying family confl ict situations since the beginning of prenatal care in order to address the issue and enable higher adherence to follow-up among victimized pregnant women. The objective of the article was to offer an operational framework to assess cross cultural adaptation processes of instruments developed in other linguistic, social and cultural contexts. It covers the need for using robust measurement tools; the importance of 'universal' instruments that permit cross cultural fi ne-tuning; and stresses the need for adapting existent instruments rather than developing new ones. Existing controversies and proposals for different procedures in current literature are reviewed and a model for adapting instruments is presented. This synthesis covers the operational steps involved in evaluating concepts, semantic and operational items, and presents psychometric analysis guidelines that underlay an evaluation of measurement equivalence. Finally, the need for adequately controlling the quality of information presented in epidemiological studies, including a meticulous cross-cultural adaptation of research agendas, is reinforced.
