Crossover assessment of serum bactericidal activity and pharmacokinetics of five broad-spectrum cephalosporins in the elderly

Abstract: To better define the pharmacokinetics and serum bactericidal activity (SBA) of the expanded-spectrum cephalosporins in the elderly, we administered single 2-g intravenous infusions of cefoperazone, cefotaxime, ceftriaxone, ceftazidime, and ceftizoxime to six healthy volunteers over the age of 65 years. Serum was collected over 24 h, and concentrations were determined by high-performance liquid chromatography; pharmacokinetic parameters were determined for each drug. SBA was measured against representative stra… Show more

“…There are few data on the optimal dose of ceftriaxone in the elderly, and current guidelines do not recommend alterations to the dosing regimen in elderly patients. Our data accord with a small study of six elderly patients with a mean eGFR of 55 ml/min/ 1.73m 2 that identified a large difference between observed drug clearance in the group of elderly patients compared with that in younger healthy volunteers (19). In contrast, another study did not show as large a difference, but the recruited elderly volunteers had a higher mean eGFR (6).…”

“…As an alternative to conventional susceptibility tests, the serum bactericidal activity (SBA) test, which can take protein binding into account, may provide a more meaningful measure of the potential in vivo activity of an antimicrobial agent (9,36). The present study assessed the levels of protein binding of vancomycin and teicoplanin in the serum of human volunteers.…”

“…Previously reported total drug levels do not account for the approximately 20% of bound drug that fails to precipitate with methanol or does not diffuse freely in agar. The combination of undetected, bound drug and concentration-dependent shifts in free drug levels might contribute to the difficulty some investigators have encountered in trying to fit ceftriaxone pharmacokinetics to standard two-compartment pharmacokinetic models using either first-or second-order kinetics (5,10,11).…”
