Cryopreserved arterial allografts for in situ reconstruction of abdominal aortic native or secondary graft infection

Abstract: The prognosis of native or secondary aortic graft infections is poor. Aortic in situ reconstruction with CAA offers acceptable early and late results. Patients with type 1 diabetes and American Society of Anesthesiologists class 4 are at higher risk of mortality.

“…However, How et al have reported a mycotic aneurysm in the brachial artery secondary to Staphylococcus epidermidis after a case of infective endocarditis that acutely enlarged and became symptomatic rapidly [ 10 ]. In their series on cryopreserved aortic reconstruction involving 71 patients, Ben Ahmed et al showed one patient with the primary aortic infection with Staphylococcus epidermidis [ 11 ].…”

“…This incidence is highest in the presence of groin incisions for lower extremity bypass procedures, up to 10.6%, and lowest for aortoiliac grafts ranging from 0.6% to 5% [2,[7][8][9]. Endovascular device infections are rare with incidence ranging from 0.1% to 1.2% [9,10].…”

“…The most dreaded SSI is graft infection which occurs in up to 15% of vascular reconstructions with incidence differing based on location [6]. This incidence is highest in the presence of groin incisions for lower extremity bypass procedures, up to 10.6%, and lowest for aortoiliac grafts ranging from 0.6% to 5% [2,[7][8][9]. “…Aortic infection is a challenging condition. 1 Long-term antibiotherapy can result in relapse of infection. 2 In contrast, surgical revision is the best solution but is associated with early mortality rates of 7% to 18%.…”
