Cryptogenic Organizing Pneumonia: IL-1β, IL-6, IL-8, and TGF- β1 Serum Concentrations and Response to Clarithromycin Treatment

Abstract: Cryptogenic organizing pneumonia (COP) is a distinct clinicopathological entity with unknown etiology. Inflammatory cytokines play a role in the development of the disease. The present study was performed to assess the correlation between concentrations of IL-1β, IL-6, IL-8, and TGF-β1 in the serum with response to clarithromycin (CAM) treatment in patients with COP. A total of 39 patients with COP were enrolled in to this study. An oral dose of 500 mg CAM was administered to all of the patients twice daily fo… Show more

“…84 Costimulation of TGF-β with IL-6 is required for Th17 cell differentiation. 85 The TGF-β1 content in the serum of TB patients is higher than that in HC controls, and the serum level of TGF-β1 is directly related to the bacterial load and radiological severity. 86 Moreover, PBMCs from TB patients produce more TGF-β than those from LTBI under Mtb antigen stimulation.…”

“…Macrolides (clarithromycin and azithromycin) have been reported to be effective in the treatment of organising pneumonia and associated with fewer side effects and relapses, particularly in patients with normal pulmonary function 13. Inflammatory cytokines produced by alveolar macrophages including interleukin (IL)-6, IL-8 and transforming growth factor-β1 play an important role in the pathogenesis of organising pneumonia 14. Macrolides inhibit cytokine production by the alveolar macrophages decreasing their concentration in the serum and bronchoalveolar lavage in patients with organising pneumonia 14–16.…”

Macrolides inhibit cytokine production by the alveolar macrophages decreasing their concentration in the serum and bronchoalveolar lavage in patients with organising pneumonia 14–16. The anti-inflammatory effects of macrolides are the basis of their effectiveness in the treatment of organising pneumonia14–16 and other diseases including chronic obstructive pulmonary disease, cystic fibrosis and panbronchiolitis.…”

“…These cytokines are essential chemotactic factors for neutrophils and play important roles in the pathogenesis of COP[ 2 , 36 , 39 41 ]. Previous data showed potential activities of macrolides in patients with COP [ 21 26 , 40 , 41 ]. The aim of this study was to compare the results of CAM versus prednisone (PRE) in patients with OP.…”
