Purpose: Polychromatic cholesterol crystals of the anterior chamber are an interesting and unusual finding. This paper examines the different pathogenetic mechanisms leading to the formation of these clinically detectable anterior chamber crystals. Methods:Three aetiologically different cases which exhibited polychromatic crystals in the anterior chamber were reviewed. Aqueous samples were examined by wet field microscopy in all cases and additionally by electron microscopy in one of these. One enucleated globe was available for histopathology.Results: Typical highly refringent cholesterol crystals were identified in the aqueous of all cases. In the first case, the cholesterol crystals developed following the breakdown of vitreous and anterior chamber haemorrhage. In the second case, the cholesterol appeared to derive from the subretinal fluid of a chronic total retinal detachment in the absence of any intraocular haemorrhage. The cholesterol crystals of the final case resulted from phacolysis and were associated with a marked neutrophil response and the presence of proteinaceous crystals consistent with the crystallins. Conclusion:Anterior chamber cholesterolosis is a secondary phenomenon that always occurs as a result of an ocular disease process. Although the prognosis is dismal for chronically diseased eyes displaying cholesterol crystals in the anterior chamber, the prognosis for eyes with phacolysis may be excellent.

Intraocular cholesterol granuloma (CG) associated with synchysis scintillans (SS) was diagnosed in a 5-year-old spayed Shetland sheepdog. During the initial clinical examination, the patient exhibited SS in the anterior chamber. Canine SS is usually found in the vitreous cavity, and SS in the anterior chamber has not been described. Since canine SS has been reported to be a non-progressive condition, and its long-term clinical course has not been adequately documented. The present case report describes the long-term clinical course of a case of canine SS, in which SS occurred in the anterior chamber, leading to intraocular CG formation, and eventually glaucoma.

Ciddi göz içi basınç yükseklikleri ile giden fakolitik glokomlu olgularda iridesan partiküllerin mevcudiyeti iyi bilinmekte olan bir klinik durumdur. Bu hiperkromatik kristallerin kolesterol kristalleri olduğu konusunda çeşitli yayınlar mevcuttur. Fakat bazı yayınlarda bu kristallerin okzalat içerikli olabileceği üzerinde durulmuştur. Ayrıca bu kristallerin yapısını mikroskopik olarak gösteren çok az sayıda yayın mevcuttur. Bu olgu sunumunda amacımız ön kamarada yaygın kristal birikimleri olan fakolitik glokomlu bir olgunun sunulması ve bu kristallerin mikroskobik özelliklerinin ortaya konmasıdır. Nevertheless, there are few articles which provide information about these crystalline structures at the microscopic level. The purpose of this manuscript was to report the case of a phacolytic glaucoma patient with intense crystalline accumulation in the anterior chamber and to present the microscopic features of these crystalline deposits.
