CT-Derived Pulmonary Artery Diameters to Preselect for Echocardiography in COPD Patients Eligible for Bronchoscopic Treatments

Abstract: Background: Currently, patients with COPD who are evaluated for bronchoscopic treatments are routinely screened for pulmonary hypertension (PH) and systolic left ventricle dysfunction by echocardiography. Objectives: We evaluated the prevalence of PH and systolic left ventricle dysfunction in this patient group and investigated if the previously proposed CT-derived pulmonary artery to aorta (PA:A) ratio >1 and PA diameter meas… Show more

“…This could be explained by the relatively low prevalence of these comorbidities in comparison to other cohorts involving only patients with severe COPD [32,34]. This is in line with previous findings in a similar cohort as ours, herein only 1 out of 255 patients had systolic left ventricle dysfunction [35]. Diabetes and chronic kidney disease, with prevalences of 5% and 4% respectively, were also uncommon in our cohort [32].…”

“…In this study, only modest improvements in RVEDV were found in patients who had a signicant lung volume reduction (116.9 ml vs. 119.5 ml). However, echocardiography has been shown to be very challenging in patients with severe emphysema (29). Although all lung volume reduction methods resulted in increased end-diastolic volumes in these studies, a trend might be observed that larger reductions in lung volume tend to result in larger increases in RVEDVI and cardiac output based on our results and earlier studies.…”
