CT findings at lupus mesenteric vasculitis

Abstract: CT is helpful for confirming the diagnosis of LMV, especially the comb sign which may be an early sign. Bowel ischemia due to LMV is less ominous than previously expected, and the abnormal CT findings were reversible when early diagnosis and prompt i.v. steroid therapy could be achieved.

“…Enhancing ascites, bowel thickening and halo/target signs were seen. A palisade or comb like pattern representing prominent vasculature supplying dilated bowel loops was concluded to be a potential early sign of disease [109]. Gas collection in the bowel and fat attenuation can also be seen on CT scan [106].…”

“…The mesentery is often involved, with engorgement of mesenteric vessels Bcomb sign,^and increased attenuation of the mesenteric fat [3,45,46]. The area supplied by the superior mesenteric artery is preferentially affected [46], with the territory supplied by the inferior mesenteric artery also frequently involved [45]. Small bowel (jejunum, ileum) involvement is more frequent than large bowel, while gastric involvement is uncommon [47].…”

“…CT findings can include focal or diffuse bowel wall thickening with target sign bowel dilatation and, rarely, pneumatosis intestinalis. The area supplied by the superior mesenteric artery is preferentially affected [46], with the territory supplied by the inferior mesenteric artery also frequently involved [45].…”

“…Common CT findings in patients with LMV include dilated bowel, focal or diffuse bowel wall thickening, abnormal bowel wall enhancement (a double halo or target sign), mesenteric edema, engorgement of mesenteric vessels (comb sign), and ascites (Ko et al 1997;Ha et al 2000). It is usually multisegmental and not confined to a single vascular territory.…”
