CT Imaging of Early Hepatocellular Carcinoma and the Natural Outcome of Hypoattenuating Nodular Lesions in Chronic Liver Disease

Abstract: Background: The Liver Cancer Study Group of Japan has designated early hepatocellular carcinoma (HCC) a clinically early-stage HCC corresponding to a high-grade dysplastic nodule as proposed by the International Working Party. Methods: The majority of resected early HCCs were demonstrated as having hypo- or isoattenuation in the arterial and delayed phases of dynamic CT. Results: Only 5% of early HCCs were hyperattenuated on dynamic CT, whereas 94% of advanced small HCC were hyperattenuated. CT arterial portog… Show more

“…Some investigators have previously reported a correlation between histopathologic grade and blood supply to the HCC in radiological and pathologic analyses. These researchers mainly focused on precancerous lesions and early-stage HCC (4,13). However, few previous reports have examined the vascular changes that occur in the late stage of the disease and the direct relationship between arterial blood flow to the tumor and the tumor's proliferative activity (4, 7).…”

“…This result contradicts findings in other reports described above [6,14,16,30,31] . As explained above, HCC naturally acquires rich tumor vascularity in the early stages and most clinically treated HCCs represent tumor with radiological enhancement [34,35,38] . Hypervascularity is observed in the majority of HCC [36] .…”

“…Lately, HCC has been diagnosed at an earlier stage because screening programs using ultrasonography, CT, or MRI in combination with serum tumor markers are being widely employed in patients with chronic hepatitis or liver cirrhosis, who are the at-risk populations for HCC [7,8,9]. Surgical resection or percutaneous radiofrequency ablation (RFA) are used to treat early-stage HCC in patients with good liver function [10,11,12,13].…”
