Current Concepts in Therapy of Chronic Rhinosinusitis and Nasal Polyposis

Abstract: The exact pathophysiological mechanisms leading to chronic rhinosinusitis (CRS) still to a large extent remain obscure. However, recently there has been some progress in elucidating the etiology of nasal polyposis, especially regarding tissue eosinophilia as well as the role of aspirin intolerance and eicosanoid mediators. Endonasal sinus surgery has evolved to be the treatment of choice in CRS and nasal polyposis in all cases where conservative treatment has failed or resulted in only a partial or temporary r… Show more

“…Further, some patients are nonresponders after treatment with corticosteroids for 3 months and receive a functional endoscopic sinus surgery. The recourse required by these patients implies that the level of TGF-β1 is not modulated by corticosteroid treatment, indicating that corticosteroids show no effect on TGF-β1 expression in CRS, as suggested by the results of computed tomography/endoscopy, histopathologic examination, and patients’ complaints [35,36,37]. …”

“…(6,7) Surgery, often in combination with topical and systemic medical treatment, has been the primary approach for treating CRS. (3,8) An effective topical therapy may allow treatment of upper respiratory diseases prior to or after surgery and can potentially prevent surgery.…”
