Current radiodiagnostic concept of ankylosing spondylitis

Abstract: Nine radiodiagnostic rules are drawn from the X-rays of 2125 ankylosing spondylitis patients. The significance of early diagnosis of the 'multicolored' saroiliac X-ray is looked onto; the syndesmophyte, squaring-phenomenon, barrel-shaped vertebra, Romanus and Andersson lesions, and the ossification of ligaments are discussed. The changes at the apophyseal and the costovertebral joints, unimportant for early diagnosis, are explored.

“…Another problem encountered in the management of AAU patients, was the radiologic interpretation of the sacroiliac joints. Although x-ray examination of the sacroiliac joints is useful in differentiating inflammatory disease from other causes of low back pain [24] and although bilateral sacroiliitis is a conditio sine qua non for AS [25], sacroiliitis is by no means readily identifiable or easily interpretable [26]. In early stages of AS, sacroiliac joints often show a normal aspect or only minimal changes [27], while mild or moderate degrees of sacroiliitis may not interpreted correctly [28].…”

“…Radiographic sacroiliitis is by no means an early or obligate manifestation of the disease and may become manifest only after appropriate follow-up. [3][4][5][6] There is a paucity of study of enthesitis in India; hence this study was envisaged to attempt to study the profile of entheseal involvement in patients with spondyloarthropathy.…”
