Current status of isolated limb infusion with mild hyperthermia for melanoma

Abstract: ILI with mild hyperthermia is an established alternative to hyperthermic ILP for patients with recurrent limb melanoma. It can also be used for patients with soft tissue sarcomas and a variety of serious, chronic dermatological conditions.

“…Previous reports have suggested the use of regional chemoperfusion as neoadjuvant therapy to shrink tumor size and potentially to facilitate surgical resection. 11,13,20-23 Our study did not specifically evaluate the role of ILI as a neoadjuvant therapy, yet 4 patients were rendered disease free at a median follow-up of 8 months, which is a promising result.…”

“…These can be prevented by the injection of a muscle relaxant or by lifting the treated area away from the muscle for cutaneous lesions [Gehl, 2008]. There are other techniques currently available for the treatment of skin melanomas such as chemotherapy, chemo-immunotherapy, isolated limb perfusion [Grünhagen et al, 2006], and isolated limb infusion [Thompson and Kam, 2008]. However, they have lower response rates or their side effects are much more problematic, and they are technically complicated for the physicians.…”

“…[1][2][3] Although results after ILP are satisfactory, the technique involves a complex and invasive surgical procedure with a substantial risk of complications. 4,5 Isolated limb infusion (ILI) has been developed at the Sydney Melanoma Unit as a simplified and minimally invasive alternative to ILP. 6,7 In essence, ILI is a low-flow ILP performed via percutaneous catheters without oxygenation.…”
