Current Surgical Strategies for the Treatment of Rectal Adenocarcinoma and the Risk of Local Recurrence

Abstract: Background: Despite new medical and surgical strategies, 5-year local recurrence of rectal adenocarcinoma was reported in up to 25% of cases. Therefore, we aimed to review surgical strategies for the prevention of local recurrences in rectal cancer. Summary: After implementation of the total mesorectal excision (TME), surgical resection of rectal adenocarcinoma with anterior resection or abdominoperineal excision (APE) allowed decrease in local recurrence (3% at 5 years). More recently, extralevator APE was de… Show more

“…Except for colectomy to remove diseased colon permanently, current leading managements for diverticular disease including medication and endoscopic/laparoscopic surgery cannot prevent the occurrence/recurrence of diverticulitis, bleeding and/or other complications (Cirocchi et al, 2015;Yamada et al, 2015;Kaushik et al, 2016;Longchamp et al, 2021). Developing more effective minimally invasive approaches leading to less complications and lower diverticulosis recurrence are urgently needed both for patients and the healthcare system.…”

“…To achieve a successful curative resection, it is important to perform a thorough removal of the cancer by ensuring negative margins on a histological level [13]. Additionally, a total mesorectal excision (TME) should be carried out, which involves removing local lymph nodes through transabdominal procedures such as low anterior resection (LAR) or abdominoperineal resection (APR) [14,15].…”

“…Over the past four decades, there has been a dramatic improvement in the prognosis of non-metastatic RA due to advances in staging and multidisciplinary treatment processes [7]. Subsequently, staging has been enhanced using Magnetic Resonance Imaging (MRI) and Endoscopic Rectal Ultrasonography (ERU) [8].…”
