Cushing's Syndrome Associated With a Suprasellar Tumour

Abstract: A case is described of a roentgenographically confirmed primary suprasellar tumour in a 14 year old Negro boy, who presented with the clinical and laboratory findings of Cushing's syndrome, together with hyperpigmentation, homonymous hemianopia and unilateral ptosis. This appears to be the youngest case exhibiting Cushing's sydrome secondary to this aetiology, and one of a small number in whom the tumour could be well delineated roentgenographically in the presence of an essentially normal sella. The significa… Show more

“…In the present patient, the presence of subnormal responsiveness to ACTH, metyrapone and Pitressin@, absence of normal dexamethasone suppressability, absence of normal corticosteroid circadian periodicity, and initially markedly elevated plasma ACTH concentration, are in keeping with the majority of reports cited both for ACTH producing pituitary tumours (Krieger et al, 1964;Nelson et al, 1966) and ectopic ACTH producing tumours (Engel & Kehane, 1963;Liddle et al, 1963;O'Riordan et al, 1966;Friedman et al, 1966;Landon et al, 1967;Strott et al, 1968;Melvin et al, 1970;Marks et al, 1975).…”

“…The distinction may be more than academic. Of sixteen cases of 'primary' tumour examined pathologically, two were chromophobe adenomas, and the remainder pituitary carcinomas, six of which showed evidence of metastases (Krieger et al, 1964). In all the 'secondary' cases, except one reported as a transitional basophilic carcinoma (Myerson & Hingston, 1962), the pathology of the pituitary tumour has been that of an adenoma, usually chromophobe in nature.…”

“…1673 Das auf den Künstler projizierte "Außenseitertum" entwickelte sich bereits seit der italienischen Renaissance. 1674 Die Vorstellung vom künstlerisch-pathologischen Ausbruch aus den Strukturen der Gesellschaft führt zurück zur Diskussion über die "Bürgerlichkeit" (siehe Kapitel 2), wobei die radikale Abwendung der Künstler vom "Bürgertum" ab dem 19. Jahrhundert einsetzte.…”
