Cutaneous Ciliated Cyst of the Scalp

Abstract: Cutaneous ciliated cysts (CCC) are rare benign cysts known to occur in the lower extremities of females of reproductive age. Currently, there are 2 theories that attempt to explain the histogenesis of this rare entity. The theory of Mullerian heterotopia provides a plausible histogenetic explanation for the vast majority of CCC. A proposed alternative theory is the ciliated metaplasia of eccrine glands. We believe that previously reported cases of CCC include 2 distinct entities. We report, herein, the first c… Show more

“…The eccrine metaplasia theory was based partly on the immunoreactive patterns (positive for CEA and negative for hormonal receptors) of rare ciliated cutaneous cysts suggestive of eccrine differentiation. 7 The identification of ciliated cells in fetal eccrine ducts/glands 19 and in eccrine spiradenoma 20 was also cited as a histogenetic basis for the "ciliated metaplasia of the sweat gland" hypothesis. However, these observations serve at best as circumstantial evidence as a bona fide case of ciliated metaplasia in eccrine units has yet to be described in the literature.…”

“…2 Notably, the presence of ciliated cells was not reported in any series or reviews of syringometaplasia although ciliated eccrine metaplasia has been postulated as a possible pathway in the histogenesis of some cutaneous ciliated cysts in male patients 3–5 or at unusual anatomic sites. 3–8 In this report, the author documents for the first-time ciliated metaplasia in eccrine ducts and glands and discusses the pathogenetic significance of this peculiar histologic finding, particularly in the context of the histogenesis of a subgroup of cutaneous ciliated cysts.…”
