Cutaneous Metastasis at a Surgical Drain Site after Gastric Cancer Resection

Abstract: Cutaneous metastasis from intra-abdominal malignant solid tumours such as gastric adenocarcinoma is very rare. Here, we report the case of a 76-year-old male patient with a T4N2M0, poorly differentiated, signet-ring cell gastric carcinoma, who underwent potentially curative resection of the tumour and developed cutaneous metastasis at the site of the surgical drain 4 months after the operation while he was on chemotherapy. The lesion involved the skin and the subcutaneous fat only. A CT scan revealed local rec… Show more

“…We searched the PubMed database for reports of cases undergoing surgical resection of cutaneous metastasis from gastric cancer using with the keywords “gastric cancer,” “resection,” and “cutaneous metastasis.” Six cases were found (Table 1) [7, 8, 9, 10, 11, 12]. Five out of 7 patients, including ours, lived longer than 3 years after resection of the cutaneous metastasis, indicating a benefit from the surgical intervention.…”

“…The skin metastatic lesions have no specific appearance; they may present as cutaneous nodules, ulcers or persistent inflammatory lesions. 2 Clinically the differential diagnosis in the presented case may include bacterial infected lesion, foreign body granuloma and hypertrophic scar. The long permanence of percutaneous drains can be a risk in developing cutaneous metastasis at the drainage site, but this event is rare.…”

“…We searched the PubMed database for earlier case reports treated by surgical excision of subcutaneous metastasis due to gastric cancer using the key words "gastric cancer, " "cutaneous metastasis, " "subcutaneous metastasis, " and "skin metastasis. " We found six subcutaneous metastasis cases without any other synchronous metastases after curative gastrectomy (Table 3) [22][23][24][25][26][27]. The median age was 68 years (range 60-89 years).…”
