CutaneousT‐cell lymphoma in patients with chronic lymphocytic leukemia: clinical characteristics, temporal relationships, and survival data in a series of 14 patients atMayoClinic

Abstract: Patients with CTCL concurrent with or prior to CLL have a worse overall survival than patients with CLL in whom CTCL later develops. Larger studies are needed.

“…This is the first known case of coexistent cutaneous CLL and LyP within the same skin biopsy. As stated previously, these conditions have been described before in the same patient, but not concurrently in the same tissue . This raises an important question: is this patient predisposed to multiple coexisting hematologic/lymphoproliferative malignancies, or are there other complicating factors at play such as his treatment for his known CLL?…”

“…But most of these theories are far to be confirmed . Chang et al examined the temporal relationships and survival of a group of 14 patients with coexisting CTCL and CLL. They determined that in patients whose CTCL precedes the CLL, the overall survival is worse.…”

“…Indeed, the coexistence of cutaneous T-cell lymphomas (CTCL) and low grade B-cell lymphomas, most commonly chronic lymphocytic leukemia (CLL), further exemplifies the complex pathogenesis underlying the proliferation of 2 distinct lineages and reinforces that the mechanisms behind this phenomenon are likely multifactorial. Chang et al, 15 in their review of 14 patients with CTCL and CLL, presented 8 patients in which both diseases were diagnosed concurrently, 5 in which CLL was diagnosed first, and 1 in which CTCL was diagnosed first. Antecedent viral infection, previous radiotherapy for breast cancer, and eventual development of prostate cancer were but a few examples of associations seen in their patients, supporting the theory of multifaceted pathogenetic mechanisms linking multi-lineage proliferation.…”

“…[13][14][15][16][17][18] Additionally, an association of B-CLL and various cutaneous T-cell lymphomas has been documented, including MF, Sézary syndrome, peripheral T-cell lymphoma involving the skin, and natural killer-cell lymphoma. [19][20][21] Occurrence of leukemic infiltrates on the site of a previous herpes simplex or herpes zoster infection leads to the speculation that specific skin infiltrates of B-CLL can be triggered by antigenic stimulation. [2][3][4][5]22 We are aware of 2 previously published articles documenting the association of specific B-CLL infiltrates and B. burgdorferi infection.…”
