StreszczenieOpisujemy rzadki przypadek zaniedbywania i wyzyskiwania dziecka. Według wiedzy autorów podobnego przypadku nie opisano dotychczas w literaturze. Dziecko z wrodzonym zarośnięciem odbytu zostało poddane zabiegowi wyłonienia tymczasowej kolostomii w dniu urodzenia. Kolejny zabieg chirurgiczny, polegający na zamknięciu kolostomii i rekonstrukcji odbytu, zaplanowano na okres po ukończeniu przez dziecko miesiąca życia. Osoba sprawująca opiekę nad dzieckiem nie zgłosiła się jednak do przeprowadzenia zabiegu. Dziecko w wieku 12 lat zostało przywiezione na szpitalny oddział ratunkowy z nieodprowadzalnym wypadnięciem kolostomii i objawami ostrego brzucha stanowiącymi zagrożenie dla życia. W trybie nagłym przeprowadzono zabieg laparotomii i zamknięcia kolostomii wraz z rekonstrukcją odbytu. Stwierdzono, że dziecko było zaniedbywane i krzywdzone poprzez brak dostępu do podstawowego leczenia chirurgicznego, szczepień oraz edukacji. Dziecko było wykorzystywane do żebrania. Korzyści finansowe z tej działalności uzyskiwała osoba sprawująca opiekę nad dzieckiem i inne osoby. Opisany przypadek zaniedbywania i wyzyskiwania dziecka z atrezją odbytu ukazuje wady systemu opieki zdrowotnej, który nie zapewnia monitorowania i leczenia dzieci z wadami wrodzonymi w rodzinach o niskim statusie społeczno-ekonomicznym.Słowa kluczowe: krzywdzenie dziecka, zaniedbywanie, wyzyskiwanie, atrezja odbytu. AbstractWe report a unique and rare case of child neglect and exploitation and similar case was not found. An infant, born with an imperforate anus, underwent surgery for a temporary colostomy on the day of birth. Surgery for reversal of colostomy with reconstruction was planned at one month of age. However, the guardian did not ensure that the child attended for this procedure. Subsequently, at the age of 12 years, the child was presented to the emergency unit with an irreducible prolapsed colostomy and a life threatening acute abdomen. Emergency laparotomy and colostomy reversal were performed with anal reconstruction. The child had features of gross neglect and abuse in the form of deprivation of essential surgical care, vaccination, and education. He was used for the purpose of begging for the financial benefit of his guardian and others. This is a case of neglect and exploitation of a child with imperforate anus and it emphasizes the failures of the health system to manage and follow-up children with congenital defects born to families with a poor socio-economic background.Key words: child abuse, neglect, exploitation, imperforate anus. Purpose -The purpose of this paper is to report on the findings from a study commissioned by the Office of the Children's Commissioner (OCC) in England, concerning intrafamilial child sexual abuse (IFCSA)/incest. Specifically, it aims to explore what is known about the prevalence, nature, and impact of IFCSA and where the gaps in knowledge lie.Design/methodology/approach -A rapid evidence assessment (REA) was used, the function of which is to: search the literature as comprehensively as possible within given time constraints; collate descriptive outlines of the available evidence on a topic and critically appraise it; sift out studies of poor quality; and provide an overview of the evidence. Over 57,000 documents were scanned, and 296 ultimately systematically analysed.Findings -It was found that: there is wide variation in prevalence rates between studies; girls are more likely to be victims than boys; the onset of abuse is typically school age; abuse in minority groups is underreported; sibling abuse may be more common than that by fathers; female perpetrated abuse may be under-reported; families where abuse occurs are often dysfunctional; and IFCSA has significant adverse effects on victims.Research limitations/implications -A REA is not a full systematic review, differing in the scope and depth of the searches and depending almost exclusively on electronic databases, not accompanied by searching journals by hand.Practical implications -This work found numerous gaps in current knowledge about IFCSA, which the authors recommend be addressed by further research, including: the scale and nature of IFCSA in disabled victims, research on BME children's experiences; the prevalence of abuse by stepfathers as compared to biological fathers; the experiences of male victims; the experiences of lesbian, gay, bisexual, and transgendered child victims; the short-term impact of IFCSA based on child victims' experiences; and more widely, further research on the prevalence of abuse in clinical populations and the relationship between that and prevalence in wider society. In addition to such questions, the OCC inquiry will also investigate issues surrounding child protection and criminal justice responses to (IF)CSA and how these might be improved. The evidence base for this section of the inquiry is reported in Gekoski et al. (2016).Originality/value -The findings of this research provide the evidence base for a new twoyear inquiry into the subject of IFCSA by the OCC.

Çocuklara yönelik cinsel istismar; psikososyal gelişimini tamamlamamış çocuğun bir yetişkin tarafından cinsel uyarılma için kullanılmasıdır. Uygun şekilde değerlendirilmeyen cinsel istismar olgularında, çocuğun korunması engellenebileceği gibi sanığın ceza alması veya suçsuz bir kişinin zarar görmesine de neden olunabilir. Çalışmada; Kocaeli Üniversitesi Tıp Fakültesi Adli Tıp Anabilim DalTna başvuran ve cinsel istismara uğradığı bildirilen olguların özellikleri ve muayene süreçlerinin değerlendirilmesi amaçlanmıştır. 1998-2006 tarihleri arasmda Kocaeli Üniversitesi Tıp Fakültesi Adli Tıp Anabilim DalTna başvuran adli olgulara ait rapor ve dosyalar retrospektif olarak incelenmiş olup, cinsel istismar kapsamına giren 50 (%3,1) olgu çalışma kapsamına alınmıştır. Olguların %62’si kadın, %38’inin erkek olduğu, %60’ının 12 yaşından küçük yaş grubunda bulunduğu saptanmıştn. Sanıkların genellikle mağdurun tanıdığı kişiler (%70) ve tümünün erkek olduğu saptanmıştn. Olguların %36(18)’smda fiziksel bulgulara yönelik sorular sorulurken, olguların %50 (n:25)’sinde ruh sağlığının, %14 (n:7)’üne ise fiziksel ve ruhsal değerlendirilmesine yönelik sorular sorulduğu görülmüştür. Ruh sağlığı açısından değerlendirilen 38 olgunun %60,5(n:23)’inde cinsel istismarla ilişkilendirilebilecek ruh sağlığında bozulma saptanmıştır. Yeni yasal düzenlemeler sonrası Adli Tıp Anabilim Dalına yansıyan cinsel istismar olgularında önemli artış olduğu belirlenmiştir. Adalet Bakanlığı ile Sağlık Bakanlığı’na bağlı birimlerin mevcut yapılanma ve işleyişleri, cinsel istismarların değerlendirilmesi için uygun görülmemektedir. Cinsel Suçları Değerlendirme Merkezlerinin kurulması, tanı konulması yanı sıra ortaya çıkan veya çıkabilecek patolojilerin tedavilerinin de gecikmeden yapılması da sağlanmış olacaktır.Anahtar kelimeler: Çocuk, cinsel istismar, ruhsal muayene, çocuk ruh sağlığı ve hastalıkları

Abstract: The data support the notion of a victim-to-victimiser cycle in a minority of male perpetrators but not among the female victims studied. Sexual abuse by a female in childhood may be a risk factor for a cycle of abuse in males.
