Cyclophosphamide metabolism in children with Fanconi’s anaemia

Abstract: Summary:Although patients with Fanconi's anaemia (FA) exhibit a heightened sensitivity to DNA cross-linking agents, modified doses of CY continue to be used in their conditioning prior to BMT. We measured the pharmacokinetics and metabolism of CY in six children with FA using an established high performance thin layer chromatography technique. CY doses ranged between 5 and 20 mg/kg (median 10 mg/kg). The median CY clearance was 0.6 l/h/m 2 (range 0.4-1.1 l/h/m 2 ), t 1/2 was 8.1 h (range 6.7-9.5 h) and volume … Show more

“…41 --43 CY Cl (L/h/m 2 ) was significantly lower in our patient population compared with three studies in the pediatric population treated with myeloablative doses of CY (Table 3). CY CL has been shown to be reduced in patients with hepatic dysfunction 44 and also in patients with Fanconi anemia 45 receiving lower than the myeloablative dose of CY. It is possible that the influence of age on CY CL in thalassemia and Fanconi anemia is masked by other factors including the disease itself.…”

“…It is a prodrug requiring metabolic transformation to generate active alkylating species (9,10). The initial activation is mediated by hepatic cytochrome P-450 enzymes and is the major pathway of elimination of the parent drug (11).…”

“…Cyclophosph- amide pharmacokinetics was assumed to follow a single compartment open model with first order elimination. Although a two-compartment model has been reported previously for cyclophosphamide, a one-compartment model has been found to be adequate in pediatric studies (10). Estimates of pharmacokinetic parameters were obtained by a maximum-likelihood method using ADAPT II (16).…”

“…A high degree of interpatient variability has been reported in the pharmacokinetics and systemic exposure of CPA in children and adults with cancer (Moore et al, 1988;Yule et al, 1996;Ren et al, 1998;Yule et al, 1999a). This variability could partially reflect differences in the expression of individual P450 isozymes.…”
