Cylindroma Treated by Mohs Micrographic Surgery

Abstract: Cylindroma, a rare benign adnexal neoplasm, may occur in areas in which simple excision is either not possible or severely disfiguring. We present a case of cylindroma that was treated by Mohs micrographic surgery. Postsurgical evaluation of the tumor showed that the tumor had invaded a larger area than was initially suspected based on clinical examination alone.

“…IN A LANDMARK article in 1974, Tromovitch and Stegman 1 reviewed their experience utilizing the fresh tissue technique of microscopically controlled excision of basal cell carcinoma. Since that time, this technique has been used to successfully treat a variety of skin neoplasms, including basal and squamous cell carcinoma, malignant melanoma, dermatofibrosarcoma protuberans, angiosarcoma, 2 extramammary Paget's disease, 3 adenoid cystic carcinoma, 4 malignant schwannoma, 5 cylindroma, 6 microcystic adnexal carcinoma, 7,8 Merkel cell carcinoma, 9 and leiomyosarcoma. 10 However, the interpretation of frozen sections presents a challenge in certain tumor types, including melanocytic, spindle cell, and vascular neoplasms.…”

“…Staged surgical excision and carbon dioxide laser ablation may be used for multiple ES. MMS has been used to treat cylindromas and malignant ES, but we are not aware of any reports of it being used to treat nonmalignant ES. Our patient had recurrent disease at multiple sites and hence had a significant risk of subclinical disease extension, as demonstrated on MMS histology.…”

“…MMS has provided interesting insight into the true extent of these tumors. In one case reported by Lo and colleagues, a sporadic cylindroma in the external auditory canal that measured 8 mm in diameter took five stages to achieve clearance 21 . A malignant cylindroma of the scalp that had been removed after 10 stages of MMS 6 years before subsequently recurred, resulting in an 11‐stage Mohs procedure 22 .…”

“…A malignant cylindroma of the scalp that had been removed after 10 stages of MMS 6 years before subsequently recurred, resulting in an 11‐stage Mohs procedure 22 . Certainly within the context of sporadic cylindromas, MMS technique may offer the highest chance of clearance, given the recurrence rate suggested by Crain and Helwig, at 42% after excision, 7 although the treatment of multiple cylindromas using Mohs would be a prodigious task 21 …”
