Cystic tumor of the liver without ovarian-like stroma or bile duct communication: two case reports and a review of the literature

Abstract: We report two cases of cystic neoplasm of the liver with mucinous epithelium in which both ovarian-like stroma and bile duct communication were absent. The first case was a 41-year-old woman. She underwent right trisegmentectomy due to a multilocular cystic lesion, 15 cm in diameter, with papillary nodular components in the medial segment and right lobe. Histologically, arborizing papillae were seen in the papillary lesion. The constituent neoplastic cells had sufficient cytoarchitectural atypia to be classifi… Show more

“…Given the suspected BCT, image-guided needle aspiration cytology or biopsy is not implemented, due to high false negative results and the risk of peritoneal dissemination (23). Furthermore, an elevated bilirubin concentration in the aspiration fluid cannot be considered as a conclusive parameter to discriminate bilomas from BCTs with bile duct communication (24,25). In the present case study, intraoperative detection of scattered papillomas highlighted our suspicion of BCT.…”

“…Since the presence of ovarian stroma was accepted as a prerequisite for the diagnosis of BMCNs, the distinct entity of biliary intraductal papillary mucinous neoplasms was proposed [ 4 ]. However, more than a few reports of cystic liver tumors have been published where both ovarian stroma and bile duct communication are not detected [ 5 ].…”

“…Not surprisingly, case reports of female patients predominated ( 1 , 11 ). However, emerging literature describes a second variety of tumor without a mesenchymal layer, but rather a hyaline, fibroid, or myxoid stroma ( 2 , 14 ). Neither the case we present, nor 10 of the 11 previously described cases of MCA in children contained ovarian-like stroma.…”
