Cysticercosis of tongue: Cytohistologic approach to diagnosis

Abstract: Cysticercosis is continuing to be a major health problem in developing countries. Radiological and serological techniques are routinely used for pre-operative diagnosis of cysticercosis. But fine needle aspiration cytology (FNAC) is cost effective and simple procedure, so it is important to be aware of diagnostic pitfalls in the cytomorphologic diagnosis of cysticercosis. We present a case of cysticercosis of tongue, which accounts for only 34 cases in the world literature.

“…Diagnosis of cysticercosis is made on fine needle aspiration cytology (FNAC) only when fragments of metacestode tegument and parenchyma are identified, which were absent in the present case. Even to suspect a parasitic infection characteristic cell type host inflammatory reaction consisting of eosinophils, neutrophils, palisading histiocytes, and giant cells must be present in the aspirate from suspicious subcutaneous nodule (Koteeswaran et al 2013). Cytochemical evaluation of cerebrospinal fluid is also of great help in extra parenchymal cysts, showing various stages of inflammatory cell reaction (increase of lymphocytes, proteins, and hypoglycorrhachia) that are important to perform prognosis before antiparasitic treatment.…”

“…[1][2][3][4][5][6][7][8] Although tongue has abundant muscular tissue, it is not a common site for cysticercus due to its high muscular activity and metabolic rate which prevent the development of larva. 6 The background usually comprises of mixed inflammatory cells infiltrate consisting of neutrophils, eosinophils, lymphocytes, and histiocytes surrounding the fibrillary material, occasionally granulomas may also be seen. 11 Parakh et al 12…”

“…Oral cysticercosis is very rare in humans. On extensive search of literature, only 64 cases of lingual cysticercosis have been reported till date .
