Cytological diagnosis of amyloidosis presenting as a supraclavicular swelling

Abstract: Amyloidosis is a well-recognized entity, that can present as a systemic disease, or, uncommonly, as a localized mass. We report here, a rare presentation of amyloidosis presenting as a supraclavicular swelling, diagnosed on fine-needle aspiration cytology. Subsequent bone marrow examination led to the diagnosis of multiple myeloma. This case highlights the need to be aware of cytological characteristics of amyloidosis, which can have unusual presentation posing a diagnostic dilemma.

“…In such cases, a very high index of suspicion and awareness of the cytologic features of amyloid is needed, to make the accurate diagnosis of amyloid. 34 In the thyroid aspirates, amyloid can be mistaken for colloid. However, in thyroid aspirates, index of suspicion for detection of amyloid is higher, due to possibility of involvement by medullary thyroid carcinoma.…”

“…For example, in the FNA of neck lymph nodes/masses, amyloid can be mistaken for keratin or necrotic material, raising the possibility of metastatic squamous cell carcinoma. In such cases, a very high index of suspicion and awareness of the cytologic features of amyloid is needed, to make the accurate diagnosis of amyloid 34 . In the thyroid aspirates, amyloid can be mistaken for colloid.…”
