Cytomegalovirus-Associated Splenic Infarction in a Young Female Patient

Abstract: Cytomegalovirus (CMV) infection generally causes asymptomatic infection in the majority of immunocompetent individuals. However, the presentation may be complicated by life-threatening conditions in immunocompromised patients. We report a case of a 23-year-old healthy Caucasian female with acute CMV infection and splenic infarction. Serological studies confirmed acute CMV infection, and echocardiography did not show any evidence of endocarditis or mural thrombosis. We did not consider antiviral and anticoagula… Show more

“…Recently, splenic infarction has been described in infections such as malaria,[ 9 ] Epstein bar virus,[ 10 ] and cytomegalovirus. [ 11 ] The exact mechanism for splenic infarct in infectious diseases is still obscured but proposed presumptions include transient hypercoagulable state, vascular occlusion due to increased circulating immune complexes and cellular stacking, and imbalance between blood supply and demand of large hypercellular spleen. [ 12 ]…”

“…Individuals with a healthy immune system might not even experience the symptoms of HCMV infection or rarely experience noticeable symptoms (Biswas & Kansal, 2023). They often have CMV mononucleosis which can lead to several symptoms including fatigue, muscle aches, headache, sore throat, and swollen lymph nodes (Ishii et al, 2019;Pakkiyaretnam et al, 2020). Individuals having congenital (present at birth) HCMV means that the virus passed from mother to the fetus and infants who got infected shortly after birth merely through breastfeeding can experience symptoms related to a number of diseases including anemia, jaundice, hepatomegaly, microcephaly, hearing loss, seizures, rash (Lin et The available antiviral therapies against HCMV-related diseases include ganciclovir, foscarnet, cidofovir, letermovir, maribavir, and other antibody therapies (Huntjens et al, 2023).…”
