Cytomegalovirus Colitis Following Immunosuppressive Therapy for Lupus Peritonitis and Lupus Nephritis

Abstract: Wereport a womanwith lupus nephritis complicated with lupus peritonitis and cytomegalovirus (CMV)colitis. Diagnosis of lupus peritonitiswas made by abdominal computed tomography scan, colonoscopy, and ascitic fluid analysis. Steroid and cyclophosphamide therapy resulted in the improvement of severe lupus nephritis and peritonitis. “…The knowledge of this particular patient´s immune status regarding CMV is indeed relevant because of the theoretical risk of subsequent viral reactivation from latency associated with persistent low CD4 + T-cell counts [29,30]. CMV enteritis and colitis are frequently observed in immunocompromised patients with severe depletion of CD4 + T cells [31]. In our patient, the anti-CMV IgG curve would suggest a CMV reactivation [32], although it would be subclinical since no significant clinical changes were observed by the time of the curve peak.…”
