Cytomegalovirus infection presenting as acute periodontal infection in a patient infected with the human immunodeficiency virus

Abstract: During childhood, many people acquire primary infection with cytomegalovirus (CMV), one of the herpes viruses. If they later become immunosuppressed, such as occurs with human immunodeficiency virus (HIV) infection, CMV is likely to become reactivated. Severe disease caused by CMV is life-threatening in the HIV-infected population. CMV retinitis, gastritis, colitis, pneumonia, encephalitis and hepatitis have all been reported, but oral lesions due to infection with CMV are rarely reported. We report a case of … Show more

“…HCMV DNA was identified in 81% of HIV-associated periodontitis lesions and in 50% of the non-HIV periodontitis lesions, and was the most common herpesvirus identified [22]. In HIV-infected individuals, HCMV has also been implicated in acute periodontitis [23], periodontal abscess formation and osteomyelitis [24], and refractory chronic sinusitis [25]. Human herpes virus 8 was detected in periodontitis lesions of 24% of HIVinfected individuals with no clinical signs of Kaposi's sarcoma, but was not identified in any periodontitis site of non-HIV-infected individuals [26].…”

“…These ulcers can appear on any mucosal surface and may be confused with aphthous ulcers , necrotizing ulcerative periodontitis (NUP), (Dodd et al, 1993), and lymphoma. Unlike aphthous ulcers, however, which usually have an erythematous margin, CMV ulcers appear necrotic with a white halo, (Langford et al, 1990).…”
