Cytomegalovirus Pneumonia: Community-Acquired Pneumonia in Immunocompetent Hosts

Abstract: Cytomegaloviruses (CMVs) are DNA viruses and are members of the Herpesviridae family. Morphologically, CMV resembles other herpes viruses, particularly herpes simplex viruses, with important cytopathic differences. CMVs are recognized pathogens that cause community-acquired pneumonia (CAP) in compromised hosts, such as, those who undergo transplants and those on immunosuppressive drugs or steroids. This article discusses the clinical presentation, diagnosis, and therapy for this viral CAP.

“…Although the 13 H1N1 (+) patients were all lymphopenic and could facilitate CMV infection predisposition some authors claims that the temporary presence of relative lymphopenia that usually the virus H1N1 causes does not significantly alter the quality of the immune status of the patients with H1N1 acute respiratory distress syndrome [15] On the other hand Corona-Nakamura Al et al, reported that lymphopenia showed relative risk of 14.75 times for the appearance of CMV infection [16].…”

“…In cases with immunodeficiency, CMV infection can cause pneumonia, retinitis, hepatitis, encephalitis, demyelinating polyneuropathy, myocarditis, pancreatitis, gastrointestinal diseases and adrenal diseases, and so on [13,14]. On the contrary, severe life-threatening infection induced by CMV, including CMV pneumonia, is handful in immunocompetent individuals [4][5][6]. In this paper we describe a case of CMV pneumonia, associated with BOOP in an immunocompetent host, and effectively treated with ganciclovir.…”

“…CMV pneumonia often presents as severe viral pneumonia [6], with diffuse interstitial infiltrates on chest X-ray and respiratory failure [5]. The laboratory diagnosis may be difficult, and are made serologically or pathologically.…”
