Cytopathologic features of an unusual case of multiple eccrine spiradenomas misdiagnosed as a malignant round cell tumor

Abstract: A 28-year-old lady presented with multiple swellings in her left shoulder, associated with intermittent pain since last one and a half years. Radiologic imaging revealed multiple, well-defined, subcutaneous lesions in her left supraclavicular region. Fine needle aspiration cytology (FNAC) smears were initially reported as Ewing sarcoma, elsewhere. On review, the smears showed cohesive clusters of round-to-oval cells with scant cytoplasm, which were focally arranged in an acinar-rosetting pattern around hyaline… Show more

“…Diagnosis is established by skin biopsy which reveals the concomitant presence of large, closely packed cells at the central part of the tumor and smaller cells with compact nucleus at the circumference. Immunochemistry shows expression of CK, CEA, EMA, and S-100 by the tumor cells [9,10].…”

“…Only a few reports of US or magnetic resonance imaging (MRI) characteristics of eccrine spiradenoma have been published. To the best of our knowledge, there have been 3 case reports regarding the US features and 6 case reports regarding the magnetic resonance imaging features . Therefore, the purpose of this retrospective series was to analyze and describe its US characteristics.…”

“…In this case, the site as well as presence of lymphoid cells favoured a diagnosis of ES. Hyaline globules with basaloid cells on smears can also suggest the possibility of adenoid cystic carcinoma and similarly basaloid cells in acinar‐rosetting arrangement can be misdiagnosed as Ewings sarcoma . The cells associated with adenoid cystic carcinoma are monomorphic in contrast to epithelial‐myoepithelial‐lymphocytic cells in ES and cells of Ewings sarcoma show coarse nuclear chromatin .…”

“…Hyaline globules with basaloid cells on smears can also suggest the possibility of adenoid cystic carcinoma and similarly basaloid cells in acinar‐rosetting arrangement can be misdiagnosed as Ewings sarcoma . The cells associated with adenoid cystic carcinoma are monomorphic in contrast to epithelial‐myoepithelial‐lymphocytic cells in ES and cells of Ewings sarcoma show coarse nuclear chromatin . Hence, good clinical examination, proper sampling from multiple sites and careful attention to nuclear features as well as the background can prevent the false‐positive diagnosis of malignancy.…”
