Eccrine spiradenoma is an uncommon and benign adnexal tumor characterized by one or more nodules. The present article refers to a young patient with multiple and gradually enlarged nodules which remained undiagnosed and untreated for many years. Lesions were initially considered as multiple leiomyomas whereas skin biopsy revealed the existence of uncommon and benign tumors of eccrine and apocrine grandular adnexa confirming the diagnosis of eccrine spiradenoma. Similarities in clinical appearance and symptoms between tumors may lead to erroneous diagnosis and skin biopsy is an essential tool for differential diagnosis.

The purpose of this series was to evaluate the features of eccrine spiradenoma on ultrasonography (US). We reviewed the clinical data of 8 patients with eccrine spiradenoma who underwent preoperative US at 4 different medical institutions from 2004 to 2016 and analyzed the US features in terms of the tumor location, size, shape, margin, echo texture, echogenicity, posterior acoustic enhancement, calcification, septum, and color Doppler flow. There were 7 female patients and 1 male patient. The mean patient age was 45.6 years (range, 28-60 years). Most of the tumors were located primarily in the subcutaneous fat layer. The mean size of the tumors was 14.3 mm. The masses had a lobular appearance in 7 patients and had a tractlike structure in 3 patients. In 6 patients, the masses had a heterogeneous echo texture. Six cases showed hypoechogenicity with more hypoechoic foci in the masses, and 2 cases showed hypoechogenicity only. Color Doppler flow was evaluated in 7 patients; the blood flow was central and peripheral in 4 patients and only peripheral in 3 patients. All cases showed posterior acoustic enhancement and had well-defined margins. Calcification and septa were not seen in any cases. Eccrine spiradenoma is usually located in the subcutaneous fat layer, has a well-defined margin, a lobulated appearance, occasionally with a tractlike structure, a heterogeneous echo texture, a hypoechoic appearance with internal hypoechoic foci and posterior acoustic enhancement, and shows blood flow in the peripheral portion, with or without blood flow in the central portion.

FNAC, eccrine spiradenoma, multipleEccrine spiradenoma (ES) is an uncommon benign adnexal neoplasm arising from intra-dermal part of the duct of eccrine gland. 1 The committed sites are trunk, extremities and head and neck. 1 It usually presents as a tender dermal solitary nodule and rarely as multifocal with a linear zosteriform or blaschkoid distribution. 2 There are a few isolated references of multiple ES diagnosed on histopathology and only a single documented case on cytology. This case of multiple ES with a zosteriform pattern; diagnosed on fine needle aspiration cytology (FNAC) is reported for its rarity and classical clinical as well as cytological features.A 35-year-old woman presented with painful, gradually increasing swellings on the right lower extremity since approximately ten years. E: Cluster of matured lymphocytes (Giemsa stain, 4003) She gave a history of excision of similar lesions in the past, details of which were however not available. The patient was concerned that the lesions had increased in number and size lately.Physical examination revealed multiple, discrete, well-circumscribed, subcutaneous nodules in a linear dermatomal distribution over the right calf region ( Figure 1A). The nodules ranged from 0.5 to 2 cm in size and were firm, tender and fixed to the skin. Few lesions also showed slight bluish hue of the overlying skin. Clinical impression was multiple lipofibromas. FNAC was performed from multiple nodules. Papanicolaou and Giemsa stained smears were prepared which were moderately cellular, comprising of cells in cohesive clusters, sheets, and acinar pattern ( Figure 1B). At places, cells were arranged around metachromatic/magenta spherical hyaline bodies ( Figure 1C). The cells were basaloid with scant cytoplasm, round to oval nuclei and inconspicuous nucleoli ( Figure 1D).Background showed scattered spindle cells with darker nuclei resembling
