Daptomycin-Induced Eosinophilic Pneumonia Mimicking Multifocal Pneumonia

Abstract: A 71-year-old female presented to the emergency department with worsening dyspnea, dry cough, malaise, weight loss, fever, chills, and diaphoresis for one week. The patient had been hospitalized four weeks prior with right knee methicillin-resistant Staphylococcus aureus (MRSA) bursitis and was initially treated with IV vancomycin but was switched to IV daptomycin at the time of discharge for convenience of dosing. On presentation to the ED, vitals were normal. Physical examination revealed bilateral scattered… Show more

“…The findings revealed a significantly higher incidence of hypotension, hypoxemia, and injection pain in children administered with propofol alone (group P) compared to those in children administered only with remimazolam (group R) or the combination of remimazolam and propofol (group RP). This discrepancy may be linked to the potent respiratory and cardiovascular inhibitory effects of propofol 23 . Conversely, group R exhibited a higher incidence of nausea and vomiting, likely attributable to the common side effects of remimazolam.…”

“…The proposed pathophysiology of daptomycin-induced AEP involves macrophages presenting the drug or drug-hapten complex to a T-helper cell, which triggers the release of interleukin-5 (IL-5) [7,8]. Macrophages also release a chemokine known as eotaxin.…”

“…The presentation of AEP in general has a wide range of differential diagnoses, including eosinophilic granulomatosis with polyangiitis (formerly Churg-Strauss syndrome), Goodpasture's syndrome, fungal, parasitic, or bacterial pneumonia. Malignancy is often very low on the differential [8,10].…”
