BackgroundAcute exacerbation of chronic obstructive pulmonary disease (AECOPD) aggravates the overall severity in COPD patients, resulting in severe morbidity and mortality. However, there are no objective biomarkers currently available to predict the development of AECOPD. Several studies have indicated that galectin-3 (Gal-3) is involved in diseases characterized by excessive inflammatory response and fibrosis. The objective of this study was to examine the dynamic changes of Gal-3 in acute exacerbation and convalescence phases of COPD.Material/MethodsSerum levels of Gal-3, high sensitivity C-reactive protein (hsCRP), and prohormone of brain natriuretic peptide (pro-BNP) were determined using multiplex enzyme-linked immunosorbent assay kits. Serum levels of Gal-3 in 44 patients with COPD were further analyzed and correlated with the parameters of lung function and the biomarkers of systemic inflammation.ResultsThe mean level of serum Gal-3 was significantly higher in acute exacerbation of COPD compared with the level in COPD convalescence phase (32.10±9.83 versus 29.02±8.68 ng/mL, p<0.01). Serum levels of Gal-3 positively correlated with hsCRP (r=0.354, p=0.018 for total patients) and pro-BNP (r=0.319, p=0.035 for total patients) in AECOPD. In addition, the level of Gal-3 was the highest in the current smoker group, and the lowest in the never-smoker group in either the acute exacerbation phase (33.91±3.55 versus 29.12±11.73 ng/mL, p=0.036) or the convalescence phase (30.94±3.40 versus 27.76±9.68 ng/mL, p=0.045) of COPD.ConclusionsOur results indicated that serum Gal-3 is increased in AECOPD patients, which is also positively associated with systemic inflammation and smoking in patients with COPD, suggesting that Gal-3 might be a valuable biomarker for AECOPD.

В настоящее время только BNP и NT-proBNP рекомен-дованы для использования в качестве биомаркеров для диагностики, мониторинга и оптимизации фармако-терапии пациентов ХСН. Известно, что основным тригге-ром для продукции натрийуретических пептидов являет-ся натяжение стенки миокарда ЛЖ в ответ на перегрузку объе мом. Однако, кроме напряжения стенки миокарда, в патогенезе ХСН также принимают участие и другие про-цессы, такие как воспаление и фиброз. Натрийуретические пептиды не дают возможности оценивать выраженность данных процессов, в связи с чем поиск биомаркеров фиброза и воспаления является актуальным.В исследованиях DEAL-HF и СOACH была продемон-стрирована способность нового биомаркера галектина-3 отражать процессы воспаления и фиброза, лежащих в осно-ве ремоделирования миокарда у пациентов с ХСН [1][2][3].Галектин-3 принадлежит к семейству β-галактозид-связывающих протеинов. No significant correlation was found with other clinical indexes, such as systolic BP, diastolic BP, heart rate, BMI, 6-min walk test, left ventricular myocardial mass index, glucose concentration, total cholesterol, and GFR. A moderate correlation was observed for levels of NT-proBNP and plasma galectin-3 (r=0.3; p=0.02). Post-treatment decreases in galectin-3 were observed in 84.3% patients. Conclusion. Galectin-3 can be used as an auxiliary diagnostic biomarker for CHF.

The metabolic product of type I collagen synthesis, intact procollagen type I N propeptide (intact PINP), is a potential marker of bone formation and osteoporosis, which is not affected by kidney function. We sought to establish a chemiluminescent immunoassay method for the detection of serum intact PINP with previously prepared paired monoclonal antibodies and to evaluate the diagnostic value of the assay in osteoporosis. Using the capture molecule and monoclonal antibody as detection molecule, a diagnostic reagent was developed to detect intact PINP in serum with magnetic nanosphere carriers by the chemiluminescence method, and its analytical performance in the laboratory was evaluated. Serum intact PINP was measured in 142 healthy people and 115 osteoporosis patients. Results were matched with results of a similar test kit, Roche total PINP Elecsys Chemiluminescent Immunoassay Assay. Compared with the performance of the Roche PINP assay product, our method had higher sensitivity (0.02 ng/mL), wider linear range (0.02-1500 ng/mL), and anti-interference. Serum intact PINP values in osteoporosis patients were significantly higher than in healthy subjects (p < 0.001). Our method had good consistency compared with the Roche PINP assay (r = 0.9794). This chemiluminescence method for detecting serum intact PINP (CLIA-intact PINP) with magnetic nanosphere carrier technology meets the requirements of a clinical testing reagent and is expected to have clinical application after further evaluation and can compete with expensive imported kits on the market.
