Previously, inguinal hernia surgery was based exclusively on repairing the abdominal wall defects using the patient's own tissues, which were put in contact with and tensioned to recalibrate the natural orifices. At present, inguinal hernia surgery is based almost solely on mounting an allograft, which has the role of strengthening the weakened groin region that allowed the herniation. This modern method of operation on inguinal hernia can be performed in a classic or laparoscopic manner. The mesh is made of polypropylene, which is a polymer of cyclic hydrocarbons. The aim of the present study was to evaluate the effectiveness, biocompatibility, as well as the immediate and long-term complications in textile allografts used in open surgery of inguinal hernia repair. Another aim was to demonstrate once again the superiority of low-weight meshes with large pores by decreasing the number of complications caused by the synthetic material used, but also by a decrease in the tension on the tissues to which it was fixed. The present study included 255 cases submitted to inguinal hernia surgery. Only 1.5% required immediate reinter vention before discharge to evacuate hematoma. The short duration of hospitalization, the quality-price ratio, the good postoperative results, as well as the rapid socio-professional reintegration, render the use of polypropylene mesh in inguinal hernia surgery very attractive for patients.

Sửa chữa tái tạo các khuyết hổng toàn bộ thành bụng sau khi cắt lọc hoại tử do bỏng điện vẫn là một thách thức về mặt lâm sàng. Phẫu thuật sửa chữa thành bụng lý tưởng có nghĩa là phục hồi tính toàn vẹn của thành bụng và duy trì sức căng của thành bụng để ngăn ngừa sự xuất hiện của thoát vị thành bụng. Sử dụng các vạt da là một lựa chọn tốt khi điều trị các khuyết hổng thành bụng nhưng khó ứng dụng trong đa số trường hợp.Lưới phẫu thuật (surgical mesh) là một tấm lưới dệt được sử dụng làm giá đỡ vĩnh viễn hoặc tạm thời cho các cơ quan và các mô khác trong quá trình phẫu thuật được sử dụng rộng rãi trên thế giới. Một trong những ứng dụng phổ biến nhất hiện nay của lưới phẫu thuật là điều trị các trường hợp thoát vị thành bụng đảm bảo duy trì sức căng thành bụng ổn định, lâu dài và Polypropylene (PP) là loại lưới được sử dụng thường xuyên nhất. Tuy nhiên, hiện vật liệu này chưa được sử dụng rộng rãi trong điều trị bỏng sâu gây tổn khuyết lớn, sâu vùng bụng.Chúng tôi xin giới thiệu 01 ca tổn khuyết thành bụng do bỏng điện cao thế được điều trị thành công khi sử dụng tấm lưới phẫu thuật kết hợp trong điều trị.

Objective: To compare surgical outcome of polypropylene suture and skin staples for securing mesh in Lichtenstein Inguinal Hernioplasty Methodology: There were a total of two hundred people with an inguinal hernia who were involved in the study. They were randomly randomized to receive either polypropylene mesh or staples. Each patient voluntarily undergoes a Lichtenstein tension-free inguinal hernioplasty at the THQ Kherpur operating room. This procedure is performed under spinal anesthesia, and rigorous hygiene standards are adhered to throughout the procedure. In Group A, the anchorage of the mesh was conducted using 2/0 polypropylene suture, whereas in Group B, the anchorage of the mesh was accomplished using skin staples. Both methods were successful in achieving the desired results. The operating time, total surgical time, and wound infection rates were all compared between the groups. Results: The common age in cases of Group-X 36.52+9.41 years, and 38.63+8.76 years in Group B cases. Mean postoperative pain was 2.16+1.98 in Group-X and 1.46+1.24 in Group-Y, both of which had p values of 0.0001. Mean postoperative duration in Group-X was 56.7+5.47 and 48.54+4.38 in Group-Y, both of which had p values of 0.0001. The percentage of patients who had a post-operative wound infection was reported as 22 (22%) in Group-X and 6 (6% in Group-Y), whereas 78 (78%) in Group-X and 94 (94%) in Group-Y did not develop an infection; the p value for this comparison was 0.0011. Conclusion: When performing a Lichtenstein inguinal hernioplasty, anchoring the mesh with skin staples rather than polypropylene suture results in a reduction in both the average amount of time needed for the operation as well as the amount of postoperative pain experienced by the patient.
