DCE-MRI of the breast in a stand-alone setting outside a complementary strategy - results of the TK-study

Abstract: • DCE-MRI of the breast has a high accuracy in finding breast cancer. • The set of indications for DCE-MRI of the breast is still very limited. • DCE-MRI can achieve a high accuracy in a 'screening-like' setting. • Accuracy of breast DCE-MRI is strongly dependent on technique and reader experience. • A negative DCE-MRI effectively excludes cancer.

“…Dynamic contrast enhanced-magnetic resonance imaging (DCE-MRI) has been shown to be sensitive to changes in the physiological characteristics of tumor vasculature by providing quantifiable parameters, such as K trans (the volume transfer coefficient of contrast agent between the extracellular extravascular space (EES) and the blood plasma), K ep (a contrast agent backflux rate constant), Vp (plasma volume fraction), and V e (extravascular extracellular volume fraction). These characteristics potentially enable the use of DCE-MRI to diagnose [ 15 ] and manage solid tumors, including therapeutic effect monitoring [ 16 ].…”

“…However, this overestimation was constant in repeated examinations, and thus should be considered when interpreting the FGT% findings (Figure ). To ensure a high patient throughput in clinical practice, short MRI protocols that do not exceed 15 min are required . In this context, the acquisition of additional sequences should be kept to a minimum.…”

“…To ensure a high patient throughput in clinical practice, short MRI protocols that do not exceed 15 min are required. 27,28 In this context, the acquisition of additional sequences should be kept to a minimum. The use of anisotropic Dixon sequences with a very high reproducibility facilitates the simultaneous acquisition of MRI data suitable for both diagnosis and accurate FGT measurement.…”

“…Although MRM is internationally accepted to be the most accurate imaging tool in the detection of breast cancer, there is still a broad discussion about a general use of MRM for all women, i.e. screening especially since specificity and cost are much disputed [2], [3].…”

“…the general specificity of MRM could be raised to levels above 95% [3], suggesting that reader experience, and technical standards are prerequisites for “high-quality”- MRM [2].…”
