Decreased Serum Growth Hormone-Binding Protein in Patients with Liver Cirrhosis

Abstract: The recently characterized GH-binding protein (GH-BP) has an amino acid sequence identical to the extracellular domain of the GH receptor. Serum GH-BP reflects the amount of GH receptors, and the liver seems to be their main source. To evaluate the effect of liver disease on GH-BP, 52 patients with liver cirrhosis were studied. Serum GH-BP was measured by a binding assay with dextran-coated charcoal separation. Levels of GH-BP were correlated against the clinical state, assessed by Pugh's score. The GH-BP of 3… Show more

“…may be in a bound form in normal serum and 20-26% of GH in acromegalic serum. In our account for the endoge nous GH levels [17,18], serum GH levels of 20 or 100 (j.g/1 would require a correction factor of 0.83 or 0.56. respectively, with correction of a 5% GH-BP specific binding to 6.02 or 8.93%. respectively.…”

“…25-75 years of age (n = 30). Their data were pooled, as we had previously found similar GH-BP levels in men and women [ 16] and in aging [ 17], Three of the patients (No. 3.11 and 13) underwent an octreotide infusion test.…”

“…After consumption of the test meal, the PBC group exhibited a normal insulin response that contrasted markedly with the hyperinsulinemia seen in the HC group. The pathogenesis of insulin resistance in cirrhosis is still unknown; several researchers have failed to attribute cirrhotic hyperinsulinemia to portal systemic shunting but rather attributed it to deficits in binding and postbinding insulin target organ cells (17,29,30). Earlier work linked hyperinsulinemia with deficient hepatic insulin extraction or the shunting of portal venous blood to the systemic circulation (31).…”

“…Patients with liver cirrhosis exhibit a state of GH resistance with increased endogenous GH serum levels [2, 3, 6, 7, 8]and a blunted response of IGF-I and IGFBP-3 serum levels following exogenous GH administration [10, 11, 12]. Hepatic growth hormone receptor (GHR) expression [14]and serum growth hormone-binding protein (GHBP) levels [6, 15, 16, 17]– generated by proteolytic cleavage of membrane-bound GHR and therefore reflecting the abundance of liver GHR [18]– are reduced in patients with liver cirrhosis. Recent studies have characterized serum levels of basal IGF-I [19, 20], basal IGFBP-3 [20]and GH-induced IGF-I [21]as good predictors of residual liver function and survival in patients with liver cirrhosis.…”
