Deep Anterior Lamellar Keratoplasty for Acanthamoeba Keratitis Complicating the Use of Boston Scleral Lens

Abstract: Ophthalmologists should have a higher level of suspicion for AK in all scleral lens users and consider the use of deep anterior lamellar keratoplasty for refractory cases.

“…44 Additionally, patients taking oral and/ or topical corticosteroids which are responsible for reducing immunity have potential risk factors for developing an infection. [45][46][47][48][49] However, few cases of microbial keratitis due to poor compliance have been reported during scleral lens wear. Compliance with hygiene appears to be higher in SL wearers since their ocular disease necessitates specific ocular hygiene.…”

“…The majority of patients had poor compliance and were taking oral and/or topical corticosteroids which play a role in reducing the immune defense system. [14][15][16][17]24,[37][38][39] Acanthamoeba keratitis (AK) has been documented with ScCL use. 26,39 ScCL cause mild hypoxic changes to the cornea epithelium tissue due to decreased tear exchange and larger lens diameter.…”

“…26 Other risk factors for AK include dry eye syndrome, autologous tear use and long-term use of systemic corticosteroid. 39 The incidence of infections with ScCL is not frequent because of several factors. First, ScCL are worn during the day and there are few cases of overnight wear.…”

“…Individual case reports described microbial keratitis, 123,131 Acanthamoeba keratitis, 132 and nonulcerative keratitis 133 associated with scleral lens wear. Despite the fact that much of the literature reviewed described the therapeutic use of scleral lenses in management of significant ocular disease, a limited number of complications have been reported.…”
