In terrestrial animals, the epidermal barrier transitions from covering an organism suspended in a liquid environment in utero, to protecting a terrestrial animal postnatally from air and environmental exposure. Tight junctions (TJ) are essential for establishing the epidermal permeability barrier during embryonic development, and modulate normal epidermal development and barrier functions postnatally. We now report that TJ function, as well as claudin-1 and occludin expression, change in parallel during late epidermal development. Specifically, TJ block the paracellular movement of Lanthanum (La3+) early in rat in vivo prenatal epidermal development, at gestational days 18–19, with concurrent upregulation of claudin-1 and occludin. TJ then become more permeable to ions and water as the fetus approaches parturition, concomitant with development of the lipid epidermal permeability barrier, at days 20–21. This sequence is recapitulated in cultured human epidermal equivalents (HEE), as assessed both by ultrastructural studies comparing permeation of large and small molecules, and by the standard electrophysiologic parameter of resistance (R), suggesting further that this pattern of development is intrinsic to mammalian epidermal development. These findings demonstrate that the role of TJ changes during epidermal development, and further suggest that the TJ-based and lipid-based epidermal permeability barriers are interdependent.

Cutaneous granulomas caused by Candida guilliermondii are difficult to cure. In situ photoimmunotherapy (ISPI) is a novel method composed of local photothermal therapy and immunoadjuvant. In this study, ISPI was used the first time clinically for cutaneous granuloma caused by itraconazole-resistant C.guilliermondii. A 10-week cycle of ISPI was composed of (1) 5% imiquimod applied topically every other day and (2) irradiation of lesions with an 808-nm diode laser at Days 14, 28, 42, and 56. Here we report our first case. A patient was treated with ISPI for four cycles. After the treatment, the lesions were eliminated without recurrence during a 12-month follow-up. Our results demonstrate that ISPI can be used as an effective treatment modality for cutaneous fungal granuloma.

ÖzetFungal infeksiyonlar yüzeyel formlardan, derin ve sistemik formlara doğru geniş bir spektrumu kapsarlar. Çocuklarda, fungal infeksiyonların klinik özellikleri ve sıklıkları erişkinlere göre değişkenlik gösterir. Klinik şüphe ve doğru tanı aşamalarından sonra etkin tedavi basamağına geldiğimizde ise fungal enfeksiyonlarla olan savaşımızın dinamik bir süreç olduğunu görüyoruz Direnç gelişimi, farklı türlere daha sık rastlanması, daha etkin ama daha az yan etkili doz şemaları, yeni ilaçlar, hastalarımızı izlerken dikkat edilmesi gereken noktalar olup, bilgilerimizi güncel tutmamızı gerektiren başlıca nedenlerdir Etkene yönelik doğru antifungalin seçilmesi, uygun doz planlamasının yapılması, ve optimal hasta takibiyle kür sağlanması hem morbiditeyi azaltmakta, hem de derin ve sistemik formlarda mortaliteyi önlemiş olmaktadır. Bu yazıda, çocukluk çağında görülen özellikleri vurgulanarak, dermatofitozlar, piedra, Malessezia enfeksiyonları, Candida infeksiyonları, ve immünyetmezlik durmunuda rastlanan derin kutanöz fungal enfeksiyonların genel özellikleri ve tedavileri anlatılmıştır. Sum maryFungal infections cover a wide spectrum from superficial forms to deep cutaneous and systemic forms. The prevalance and clinical characteristics of fungal infections differ in children from adults. We realize the dynamic nature of our war against fungal infections when we reach the treatment phase after appropriate clinical suspicion and correct diagnosis. Emergence of resistance, different strains becoming more prevalant, more efficient but less toxic drug schemes, new drugs, tips on optimal follow up of our patients are the reasons why we have to keep our knowledge on antifungals up to date. Successful cure by organism based choise of the right antifungal, appropriate dose planning and optimal patient follow up resulting in cure will not only decline morbidity but also prevent mortality in deep and systemic forms. Here in this article, dermatophytoses, piedra, Malassezia infections, Candida infections, and major characteristics of deep cutaneous fungal infections in immune compromised children and their relevant treatments are discussed.
