Defense Mechanisms and Treatment Response in Depressed Inpatients

Abstract: The study investigated the extent to which defensive functioning and defense mechanisms predict clinically meaningful symptomatic improvement within brief psychodynamic psychotherapy for recurrent and chronic depression in an inpatient setting. Treatment response was defined as a reduction in symptom severity of 46% or higher from the baseline score on the Montgomery–Asberg Depression Rating Scale (MADRS). A subsample of 41 patients (19 responders and 22 non-responders) from an RCT was included. For each case,… Show more

“…Numerous studies have demonstrated that the maturity of defensive functioning is a protective factor against the development of psychological and somatic diseases [ 42 , 43 , 44 , 45 ]. Traumatic experiences can provoke a decrease in overall defensive functioning and increase the use of mental inhibition defense mechanisms, also called neurotic defenses.…”

“…Recognizing the importance of demonstrating that psychoanalytic treatments are efficacious, the same research group is now addressing the efficacy of psychoanalysis for treating chronic depression in a large, multicenter, international project, in collaboration with local and national psychoanalytic societies. Research efforts are currently underway to move beyond measuring symptomatic outcomes and focus on outcomes specific to the psychopathology underlying the disorder's maintenance, which are the targets of analytic treatment (see e.g., De Roten et al, 2021). Other research groups, such as in Zurich, are interested in the correlations and effects of analytic treatment on brain function (Wade-Bohleber et al, 2020).…”

“…The neuropsychodynamic model strongly recommends individualized procedures. It is widely acknowledged that MDD shares multidimensionality and heterogeneity, proving the crucial significance of adapting appropriate neurological and psychotherapeutic treatments to the specific condition in each case [82]. Neuropsychodynamics addresses the idea of MDD as psychosomatics of emotion modulation.…”
