In Germany, an ageing population is affected by societal and political changes due to demographic transition, e.g. by a prolonged working life for older employees. Demographic change also influences persons of higher working age with a migrant background. In 2018, 25% of all employees in Germany had a migrant background. Those affected by poor health at a higher working age can benefit from medical rehabilitation services, which aim to prevent early retirement and disabilities. So far, the utilisation of medical rehabilitation has been lower among persons of foreign nationality (often the only available proxy for migrant background), compared to that of Germans. The aim of this scoping review is to assess the utilisation of medical rehabilitation services by those with migrant background (PMB) and those without (non-PMB) and to identify the differences between these groups. We included 25 studies in our analysis, which were mainly secondary analyses of routine data and also a small number of primary studies. The results were inconsistent: studies published before 2018 showed a lower use of rehabilitation services for persons of foreign nationality compared to Germans. However, no differences were found between PMB and non-PMB in studies published in 2018 or later. PMB, as well as foreign nationals, showed poorer health before medical rehabilitation utilisation and had a higher chance of occupational disease and a lower education level. We identified a lower work-related performance, as well as barriers (e.g. information deficits) in the utilisation of rehabilitation services for groups of PMB. Our review is limited in that we cannot generalise our results to all PMB living in Germany. This is because of the heterogeneity, the limited number of studies and lack of representativeness in some studies. In many cases, studies only analyse the nationality, but they lack information about the second generation PMB. Future studies should survey the utilisation of medical rehabilitation services by migrant background rather than by nationality and focus on changes in the provision of rehabilitation measures following diversity-centred strategies.

ObjectiveTo describe psycho-oncological care structures and processes in German breast cancer centres from the perspective of the centre administration.MethodsThe findings are based on a postal survey of a representative random sample of surgical sites of certified breast cancer centres in Germany. Data were collected in 2013 and 2014. The questionnaire included questions about infrastructure, patient information standards, psycho-oncological services and aspects of organisational culture. Data analyses included frequencies, means and bivariate relationships.ResultsThe return rate was 88.3% (53 hospital sites). Psycho-oncological care is provided by permanent employees in 87%. The average number of full-time-equivalent employees (FTE) is 1.23. Most breast cancer centres engage the occupational group of psycho-oncologists for psycho-oncological care (90%), followed by the medical service (80%) and breast care nurses (78%) (multiple answers were possible). The correlation coefficient between FTEs and surgical treatments per year is not significant (r=0.292, p=0.051). Hospitals are screening every inpatient for the need of psycho-oncological support in 76% of all sites. Frequently used screening instruments are distress thermometer (19%), clinical interview (13%) and basic psycho-oncological documentation (11%).ConclusionOur data provide insights into the self-reported structural and procedural quality of psycho-oncological care in German breast cancer centres. Further research should examine patient and caregiver perspective on the psycho-oncological services provided by breast cancer centres.

Zusammenfassung Hintergrund Menschen mit Migrationshintergrund sind im Vergleich zu Menschen ohne Migrationshintergrund mit ihrer Versorgung durchschnittlich unzufriedener und weisen ungünstigere Rehabilitationsergebnisse auf. Gründe hierfür werden u. a. in einer mangelnden interkulturellen Öffnung von Rehabilitationseinrichtungen vermutet. Auch unerfüllte Versorgungserwartungen hinsichtlich der Gestaltung von Rehabilitationsangeboten können dazu führen, dass rehabilitative Angebote erst spät und mit großen Vorbehalten in Anspruch genommen werden, und auch der Rehabilitationsprozess selbst beeinträchtigt wird. Über die Erwartungen, die Menschen mit Migrationshintergrund an die Versorgung haben und darüber, wie sie sich im Verlauf der Rehabilitation verändern, ist bisher nur wenig bekannt. Ziel der vorliegenden Studie ist es, Versorgungserwartungen exemplarisch bei Menschen mit türkischem Migrationshintergrund im Rehabilitationsverlauf zu untersuchen. Methodik Die Untersuchung basiert auf leitfadengestützten Einzelinterviews mit 32 Menschen mit türkischem Migrationshintergrund zu drei unterschiedlichen Phasen des Rehabilitationsprozesses, die in fünf Rehabilitationseinrichtungen in Nordrhein-Westfalen und Schleswig-Holstein durchgeführt wurden. Die Auswertung der Daten erfolgte mittels inhaltlich-strukturierender qualitativer Inhaltsanalyse. Ergebnisse Die Versorgungserwartungen von Rehabilitand*innen mit Migrationshintergrund sind individuell sehr unterschiedlich. Viele Befragte erhofften sich von der Rehabilitation vor allem eine Verbesserung der gesundheitlichen Situation, Schmerzlinderung und Erholung. Darüber hinaus äußerten sie den Wunsch nach einer individuellen Behandlung und einer intensiven ärztlichen Betreuung während der Rehabilitation. Die zu Beginn der Rehabilitation formulierten Versorgungserwartungen verändern sich teilweise im Verlauf der Rehabilitation, werden den befragten Rehabilitand*innen zufolge aber nicht oder nur teilweise erfüllt. Gründe für die Nichterfüllung liegen u. a. in einer unzureichenden ärztlichen Betreuung während der Rehabilitation. Schlussfolgerung Aus den Befragungen werden sehr vielfältige Erwartungen von Menschen mit türkischem Migrationshintergrund an die Rehabilitation deutlich, was sich durch die Heterogenität dieser Bevölkerungsgruppe erklärt. Die Ergebnisse weisen auf Potenziale und Ansatzpunkte zur Weiterentwicklung der rehabilitativen Versorgung in einer vielfältigen Gesellschaft hin. Diversitätssensible Instrumente, die der Vielfalt aller Rehabilitand*innen Rechnung tragen können, können hierbei einen wertvollen Beitrag leisten.
